Description
Seropam is an orally administered selective serotonin reuptake inhibitor (SSRI) indicated for the treatment of major depressive episodes, panic disorder, social anxiety disorder, generalized anxiety disorder, and obsessive-compulsive disorder. It should be administered once daily, with or without food, with a recommended initial dosage of 10 mg daily. It is contraindicated in patients with known hypersensitivity to Escitalopram or Citalopram and should not be used concurrently with monoamine oxidase inhibitors or pimozide. Potential side effects include gastrointestinal effects, anorexia with weight loss, hypersensitivity reactions, dry mouth, nervousness, anxiety, headache, insomnia, visual disturbances, among others. Caution should be exercised when prescribing to special populations such as elderly patients, children and adolescents, patients with reduced renal or hepatic function, and poor metabolizers of CYP2C19. Storage conditions specify storing below 30°C and protecting from light and moisture.

- Pharmacology: Escitalopram is the pure S-enantiomer of the racemic bicyclic phthalate derivative citalopram and is at least 100 fold more potent than the R-enantiomer with respect to inhibition of 5-HT reuptake. It has no or very low affinity for serotonergic or other receptors including alpha- and beta-adrenergic, dopamine, histamine, muscarinic, and benzodiazepine receptors.
- Dosage:
- Usual dosage is 10 mg once daily for major depressive episodes
- Initial dose of 5 mg for panic disorder before increasing to 10 mg daily
- Usual dosage of 10 mg once daily for social anxiety disorder
- Initial dosage of 10 mg once daily for generalised anxiety disorder
- Initial dosage of 10 mg once daily for obsessive-compulsive disorder
- Administration: Escitalopram should be generally administered once daily, morning or evening with or without food.
- Interaction: SSRI or related antidepressants should not be started until 2 weeks after stopping an MAOI, and vice versa.
- Contraindications: Contraindicated in patients with known hypersensitivity to Escitalopram or Citalopram and concurrent use with monoamine oxidase inhibitors or pimozide.
- Side Effects:
- Gastrointestinal effects (e.g., nausea, vomiting, dyspepsia, abdominal pain)
- Anorexia with weight loss (increased appetite and weight gain also reported)
- Hypersensitivity reactions including rash, urticaria, angioedema, anaphylaxis
- Dry mouth, nervousness, anxiety, headache, insomnia
- Visual disturbances, among others
- Pregnancy & Lactation: Treatment decision should consider the potential risks and benefits for pregnant or breastfeeding women.
- Precautions & Warnings:
- Use with caution in patients with epilepsy, cardiac disease, diabetes mellitus, and other conditions specified
- Use in Special Populations:
- Elderly patients (>65 years of age): Initial dosage is 5 mg once daily
- Children and adolescents (<18 years): Seropam should not be used
- Reduced renal function: Dosage adjustment is not necessary in patients with mild or moderate renal impairment
- Reduced hepatic function: An initial dose of 5 mg daily for the first two weeks is recommended in patients with mild or moderate hepatic impairment
- Poor metabolizers of CYP2C19: Initial dose of 5 mg daily during the first two weeks is recommended.
- Overdose Effects:
- Symptoms: Range from central nervous system related symptoms to gastrointestinal and cardiovascular system effects, electrolyte/fluid balance conditions
- Management: There is no specific antidote. Establish and maintain an airway, ensure adequate oxygenation and respiratory function.
- Therapeutic Class: SSRIs & related anti-depressant drugs
- Storage Conditions: Store below 30°C temperature and protect from light & moisture. Keep the medicine out of the reach of children.
