Indications
- Relief of signs and symptoms of osteoarthritis
- Rheumatoidal disorders relief such as osseous rheumatism, ankylosing spondilitis, juvenile arthritis, and muscular rheumatism
- Acute symptomatic treatment of painful menstruation (primary dysmenorrhoea)
- Common headache and fever relief
- Symptomatic treatment of mild to moderate pain, such as muscle pain, headache, and dental pain
- Adjuvant with common cold and influenza associated with headache
Pharmacology
- Pharmacologically active enantiomer of racemic ibuprofen
- Non-steroidal anti-inflammatory drug (NSAID) with analgesic action
- Inhibits prostaglandin synthesis
- Absorbed primarily from the small intestine
- Metabolized in the liver and excreted mainly by the kidneys
- Plasma protein binding of about 99%
- Maximum plasma levels reached about 2 hours after oral administration
Dosage & Administration
- Dosage must be adjusted to the seriousness of the syndrome
- During chronic pain, the dosage must be adapted to the lowest effective dose
- Recommended dosage of 600-900 mg Dexibuprofen per day, at 2-3 divided doses
- Temporary dosage raise up to 1200 mg per day in acute disorders or exacerbations
- Maximum daily dose is 1200 mg
- Dosage for dysmenorrhea: 600-900 mg per day, at a divided dose
Interaction
- Simultaneous use of anticoagulant, hydantoine, and sulfonamide
- Interaction noticed with ticlopidine, lithium, other NSAIDs, ACE inhibitors, beta blockers, and cyclosporine
- Interacts with tacrolimus, corticosteroids, digoxin, methotrexate, pentoxyfiline, and phenytoine
- Interacts with probenefid, sulfinpyrazon, sulfonylurea, thiazide and thiazide type diuretics, and zidovudine
Contraindications
- Contraindicated in patients with hypersensitivity to Dexibuprofen or another NSAID
- Contraindicated in patients with active or suspected hemorrhage, Crohn's disease, or ulcerative colitis
- Contraindicated in patients with serious heart diseases, kidney function impairment, and liver function impairment
Side Effects
- Common side-effects include dyspepsia, diarrhea, fatigue, headache, nausea, and vomiting
- May cause abdominal pain, hypersensitivity reactions, bleeding, and ulcer
Pregnancy & Lactation
- Avoid use during pregnancy due to potential effects on the fetal cardiovascular system
- Caution advised during lactation due to passage of racimic ibuprofen into mother's milk
Precautions & Warnings
- Use with caution in patients with bronchial asthma or other chronic pulmonary diseases
- Use with caution in patients with hepatic, renal, or cardiac insufficiency and hypertension
- Use with caution in patients with systemic lupus or other autoimmunological diseases
- Use with extreme caution in patients with gastro-duodenal ulcers, ulcerative colitis, Crohn's disease, and alcoholism
Use in Special Populations
- Patients with mild to moderate liver function impairments must start with low amounts and be closely monitored
- Start amount must be reduced for patients with mild to moderate kidney function impairments
- Not licensed for use in children under 18 years of age in the UK, some countries permit such use
- Lowest effective dose recommended for elderly people
Overdose Effects
- Low acute toxicities with symptoms occurring at doses between 80 and 100 mg/kg body weight
- Symptoms include abdominal pain, nausea, vomiting, lethargy, headache, and tinnitus
- May cause moderate to serious symptoms such as flatulence, hypotension, and metabolic acidosis
- No specific antidote, treatment must be symptomatic
- Activated charcoal administration recommended in case of large quantities ingested
