Indications

  • Treatment of deep vein thrombosis, with or without pulmonary embolism
  • Prevention of thrombus formation in extra corporeal circulation during hemodialysis
  • Prophylaxis of venous thromboembolic disease in surgical and medical patients
  • Treatment of unstable angina and non-Q-wave myocardial infarction
  • Prophylaxis of venous thromboembolic disease in medical patients bedridden due to acute illness

Composition

  • Each 0.2 ml pre-filled syringe contains Enoxaparin Sodium BP 20 mg equivalent to 2000 anti-Xa III
  • Each 0.4 ml pre-filled syringe contains Enoxaparin Sodium BP 40 mg equivalent to 4000 anti-Xa III
  • Each 0.6 ml pre-filled syringe contains Enoxaparin Sodium BP 60 mg equivalent to 6000 anti-Xa IU
  • Each 0.8 ml pre-filled syringe contains Enoxaparin Sodium BP 80 mg equivalent to 8000 anti-Xa IU

Pharmacology

Low molecular weight heparin with high anti-Xa activity and low anti-IIa activity. Metabolised in the liver.

Dosage

  • Treatment of deep vein thrombosis:
    • Subcutaneously 100 anti-Xa IU/kg twice daily for 10 days
    • Subcutaneously 150 anti-Xa IU/kg once daily for 10 days
  • Treatment of unstable angina and non-Q-wave myocardial infarction: Subcutaneously 100 anti-Xa IU/kg twice daily for 2-8 days
  • Prevention of thrombus formation during hemodialysis:
    • Recommended dose is 100 anti-Xa IU/kg
    • For high risk of hemorrhage, reduce the dose to 50-75 anti-Xa IU/kg
  • Prophylaxis of venous thromboembolic disease in surgical patients:
    • Subcutaneously 2000-4000 anti-Xa IU (0.2-0.4 ml) once daily for 7-10 days
  • Prophylaxis of venous thromboembolic disease in medical patients: Subcutaneously 4000 anti-Xa IU (0.4 ml) once daily for 6-14 days

Administration

  • Injection site preparation:
    • Cleanse the area to inject with soap and water or alcohol swab
    • Avoid injecting near belly button, scars, or bruises
    • Alternate injection sites between left and right side of the stomach
  • Selecting dose:
    • Remove needle cap from syringe
    • Do not press plunger before injection to avoid medicine loss
  • Injecting:
    • Gently pinch and fold the skin for injection
    • Hold the syringe at 90° angle and insert the needle into the skin fold
    • Dispose used syringe in a sharps container

Interaction

Agents affecting hemostasis should be discontinued unless strictly indicated. Requires careful clinical and laboratory monitoring when used in combination with certain medications.

Contraindications

Known hypersensitivity to Enoxaparin Sodium, heparin, or other low molecular weight heparins. Active major bleeding and high risk of uncontrolled hemorrhage.

Side Effects

  • Haemorrhage (bleeding)
  • Thrombocytopenia
  • Elevations of serum aminotransferase
  • Skin reactions at injection sites
  • Neuraxial hematomas with concurrent use of spinal/epidural anesthesia

Pregnancy & Lactation

  • Pregnancy category B
  • No evidence of crossing placental barrier
  • Use during pregnancy only if clearly needed
  • Not known whether excreted in human milk

Precautions & Warnings

  • Administration route: Deep subcutaneous and intravascular routes for prophylactic, curative treatment, and hemodialysis. Not via intramuscular route.
  • Conditions requiring caution:
    • Increased potential for bleeding
    • History of peptic ulcer
    • Recent ischemic stroke
    • Severe arterial hypertension
    • Diabetic retinopathy
    • Neuro- or ophthalmologic surgery

Use in Special Populations

  • Elderly Patients: No dosage adjustment unless impaired kidney function
  • Renal Impairment:
    • No dosage adjustment in moderate and mild impairment
    • Caution and careful observation in patients with severe impairment
  • Hepatic Impairment: Caution should be used in hepatically impaired patients

Overdose Effects

Accidental overdosage may lead to hemorrhagic complications. Can be largely neutralized by slow i.v. injection of protamine sulfate.
