  • Enoxaparin Sodium is a low molecular weight heparin with a high anti-Xa activity and low anti-lla or antithrombin activity.

Composition

  • Enoxaparin Sodium BP 20 mg equivalent to 2000 anti-Xa III
  • Enoxaparin Sodium BP 40 mg equivalent to 4000 anti-Xa III
  • Enoxaparin Sodium BP 60 mg equivalent to 6000 anti-Xa IU
  • Enoxaparin Sodium BP 80 mg equivalent to 8000 anti-Xa IU

Mode of Action

  • High anti-Xa activity
  • Low anti-lla or antithrombin activity
  • Does not increase bleeding time
  • Primarily metabolized in the liver

Dosage and Administration

  • Subcutaneously 100 anti-Xa lU/kg twice daily for 10 days
  • Subcutaneously 150 anti-Xa lU/kq once daily for 10 days
  • Subcutaneously 100 anti-Xa lU/kg twice daily for 2-8 days
  • Subcutaneously 100 anti-Xa lU/kg
  • Patients undergoing general surgery with a moderate risk of thromboembolism
  • Patients undergoing orthopedic surgery with a high risk of thromboembolism
  • Subcutaneously 4000 anti-Xa IU (0.4 ml) once daily for 7-10 days
  • Pregnancy category B

Interaction and Contraindications

  • Discontinue agents affecting hemostasis prior to Cleven therapy
  • Discontinue agents such as acetylsalicylic acid, NSAIDs, ticlopidine, clopidogrel, dextran 40, glucocorticoids, thrombolytics, and anticoagulants
  • Known hypersensitivity to Enoxaparin Sodium, heparin or other low molecular weight heparins
  • Active major bleeding
  • High risk of uncontrolled hemorrhage including recent hemorrhagic stroke

Side Effects and Overdose Effects

  • Haemorrhage (bleeding)
  • Thrombocytopenia
  • Pain, bluish marks at injection sites
  • Skin rash at injection sites
  • Neuraxial hematomas with the concurrent use of Cleven and spinal/epidural anesthesia
  • Neutralization by protamine sulfate

Pregnancy & Lactation

  • Pregnancy category B
  • No evidence of Enoxaparin Sodium crossing the placental barrier
  • Caution in nursing mothers

Precautions and Warnings

  • Caution in conditions with increased potential for bleeding
  • Platelet counts measurement before the initiation of treatment

Pediatric Uses and Overdose Effects

  • No dosage adjustment in elderly patients unless kidney function is impaired
  • No dosage adjustment recommended in patients with moderate and mild renal impairment
  • Dosage adjustments for severe renal impairment
  • Caution in hepatically impaired patients
