Medicine Details

Indications

  • Symptomatic treatment of mild to moderately severe Alzheimer's dementia
  • Symptomatic treatment of mild to moderately severe dementia in patients with Idiopathic Parkinson's disease

Pharmacology

  • Carbamate derivative
  • Structurally related to physostigmine
  • Binds reversibly with and inactivates cholinesterase
  • Increases concentration of acetylcholine at cholinergic synapses
  • Relatively specific for brain acetylcholinesterase and butyrylcholinesterase

Dosage

  • Initial dose: Rivastigmine 1.5 mg twice a day
  • Dose titration: 1.5 mg, 3 mg, 4.5 mg, 6 mg twice a day
  • Maintenance dose: 3 mg to 6 mg twice a day
  • Re-initiation of therapy: 1.5 mg twice daily
  • Rivastigmine transdermal patch: 4.6 mg/24 hours, 9.5 mg/24 hours, 13.3 mg/24 hours

Administration

Twice a day, with morning and evening meals

Interaction

  • Exaggerates effects of succinylcholine-type muscle relaxants during anesthesia
  • Caution with anaesthetic agents
  • No concomitant use with other cholinomimetic substances
  • May interfere with the activity of anticholinergic medicinal products
  • No pharmacokinetic interaction with digoxin, warfarin, diazepam, or fluoxetine

Contraindications

Hypersensitivity to the active substance or other carbamate derivatives

Side Effects

  • Most commonly reported adverse reactions are gastrointestinal
  • Nausea (38%) and vomiting (23%)
  • Female patients more susceptible than male patients to gastrointestinal adverse reactions and weight loss

Pregnancy & Lactation

Not recommended during pregnancy; excreted into milk

Precautions & Warnings

  • Patients with sick sinus syndrome or conduction defects
  • Cholinergic stimulation may increase gastric acid secretion
  • May exacerbate urinary obstruction and seizures
  • Monitor body weight
  • Astma or obstructive pulmonary disease
  • May worsen extrapyramidal symptoms

Use in Special Populations

  • Increased exposure in moderate renal and mild to moderate hepatic impairment
  • Not recommended for use in children

Overdose Effects

  • Symptoms may include nausea, vomiting, diarrhea, hypertension, hallucinations
  • Bradycardia and/or syncope may occur
  • Conservative management recommended
  • Use of antiemetics considered in case of severe nausea and vomiting
  • In massive overdose, atropine can be used
