Indications
- Acute bacterial exacerbation of chronic bronchitis in adults
- Acute maxillary sinusitis
- Community-acquired pneumonia
- Pharyngitis or tonsillitis
- Uncomplicated skin and skin structure infections
- Acute otitis media in pediatric patients
- Treatment and prophylaxis of disseminated mycobacterial infections
- Helicobacter pylori infection and duodenal ulcer disease in adults
Pharmacology
- Inhibits microsomal protein synthesis in susceptible organisms by binding to the bacterial ribosome
- Active against most Gram-positive bacteria, Chlamydia, some Gram-negative bacteria, and Mycoplasmas
- More acid-stable than Erythromycin and has twice the activity against H. influenzae
Dosage & Administration
- Adults:
- Acute bacterial exacerbation of chronic bronchitis: 250-500 mg every 12 hours for 7-14 days
- Acute maxillary sinusitis: 500 mg every 12 hours for 14 days
- Community-acquired pneumonia: 250 mg every 12 hours for 7-14 days
- Pharyngitis or tonsillitis: 250 mg every 12 hours for 10 days
- Uncomplicated skin and skin structure infections: 250 mg every 12 hours for 7-14 days
- Children: 15 mg/kg/day divided every 12 hours for 10 days; 7.5 mg/kg every 12 hours up to 500 mg every 12 hours for mycobacterial infections
- Renal and hepatic impairment: May require decreased dosage or prolonged dosing intervals
Interaction
Contraindicated with specific medications such as HMG-CoA reductase inhibitors, gastroprokinetic agents, ergot alkaloids, antipsychotics, anti-gout agents, and certain antiarrhythmics
Contraindications
- Known hypersensitivity to clarithromycin, erythromycin, or any macrolide antibacterial drugs
- History of cholestatic jaundice or hepatic dysfunction associated with prior use of clarithromycin
Side Effects

- Abdominal pain
- Stomatitis
- Glossitis
- Headache
- Allergic reactions including urticaria and mild skin reactions
- Taste perversion
- Transient central nervous system side effects like anxiety and dizziness
Pregnancy & Lactation
- Use in neonates and children in appropriate doses
- Breast milk from mothers receiving Clarithromycin should not be given to infants until treatment is completed
- Not recommended in pregnant patients
Precautions & Warnings
- Discontinue if severe acute hypersensitivity reactions occur
- Avoid in patients with known QT prolongation or receiving drugs known to prolong the QT interval
- Consider risks and benefits when prescribing for patients with coronary artery disease
- Evaluate for Clostridium difficile associated diarrhea if diarrhea occurs
- Exacerbation of myasthenia gravis reported in patients receiving Clamycin therapy
Use in Special Populations
- Established safety and effectiveness in pediatric patients for various infections
- Not studied in pediatric patients under 6 months of age
Overdose Effects
Ingestion of large amounts may produce gastrointestinal symptoms; prompt elimination of unabsorbed drug and supportive measures are recommended
