Indications
- Adjunct to diet and exercise
- Improves glycaemic control
- Type 2 diabetes mellitus
- Inadequately controlled on Metformin Hydrochloride

Pharmacology
- Inhibition of DPP-4 enzyme
- Increased fasting and postprandial endogenous levels of incretin hormones
- Increased insulin secretion
- Decreased glucagon secretion
- Lowering of basal and postprandial plasma glucose
- Hepatic glucose production reduction
- Improved insulin sensitivity
- Increase in peripheral glucose uptake
Dosage & Administration
- Initiation based on patient's current Metformin dose
- Twice daily dosing
- Taking with or just after food to reduce gastrointestinal symptoms
Interaction
- No clinically relevant pharmacokinetic interaction observed
- Low potential for drug interactions
- No clinically relevant interactions with other oral antidiabetics
- Increase in Cmax and blood AUC of Metformin with certain co-administered medications
Contraindications
- Known hypersensitivity to Vildagliptin or Metformin Hydrochloride
- Renal disease or dysfunction
- Acute myocardial infarction
- Septicaemia
- Congestive heart failure
- Acute or chronic metabolic acidosis
- Undergoing radiologic studies with iodinated contrast materials

Pregnancy & Lactation
- Not recommended during pregnancy
- Potential risk to the foetus
- Not known whether excreted in human milk
- Not recommended for breast-feeding women

- Lactic acidosis risk
- Serum creatinine monitoring
- Special caution in elderly patients
- Liver Function Tests monitoring
- Caution in patients with jaundice or liver dysfunction
- Discontinuation before elective surgery with general anaesthesia

- Renal function monitoring in elderly patients
