Indications

  • Treatment of hypertension

Pharmacology

  • Olmesartan blocks vasoconstrictor effects of angiotensin-II
  • Hydrochlorothiazide is a thiazide diuretic affecting renal tubular mechanisms
  • Increases excretion of sodium and chloride
  • Reduces plasma volume with consequent increases in plasma renin activity
  • Increases aldosterone secretion and urinary potassium loss
  • Decreases serum potassium

Dosage & Administration

  • Usual starting dose is 20/12.5 mg one tablet once daily
  • May be titrated to two tablets 40/25 mg once daily

Interaction

  • No significant drug interactions with Olmesartan
  • Certain drugs may interact with Thiazide diuretics, e.g. alcohol, barbiturates, narcotics, antidiabetic drugs, other antihypertensive drugs, corticosteroids, lithium

Contraindications

  • Hypersensitivity to any component of the product
  • Anuria or hypersensitivity to other sulfonamide-derived drugs

Pregnancy & Lactation

  • Safety and effectiveness not established
  • Drug should be discontinued during pregnancy and lactation

Precautions & Warnings

  • Periodic determination of serum electrolytes should be performed
  • Hyperuricemia may occur in certain patients receiving thiazide therapy
  • Impaired renal function

Use in Special Populations

  • Renal Impairment Patients: Creatinine clearance should be >30 ml/min
  • Loop diuretics are preferred to thiazides in patients with more severe renal impairment
  • Hepatic Impairment Patients: No dosage adjustment necessary
  • Paediatric use: Safety and effectiveness not established
  • Geriatric use: Caution in dose selection for elderly patients

Overdose Effects

  • Olmesartan: Manifestation of overdosage would be hypotension and tachycardia
  • Hydrochlorothiazide: Signs and symptoms include electrolyte depletion, hypokalemia, hypochloremia, dehydration
