Indications
- Adjunct in the treatment of parkinsonism
- Adjuvant therapy for parkinsonism with levodopa

Description
- Exerts direct inhibitory effect on the parasympathetic nervous system
- Has relaxing effect on smooth musculature
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Higher affinity for central muscarinic receptors in cerebral cortex
- Lower affinity for peripheral muscarinic receptors
- May modify nicotinic acetylcholine receptor neurotransmission
- May lead to enhanced dopamine release in the striatum
Dosage & Administration
- Individualized dosage
- Low initial dose with gradual increase
- Different dosing for idiopathic parkinsonism, drug-induced parkinsonism, and concomitant use with levodopa
- Optimal level for total daily dose
- Dividing total daily intake into 3 or 4 doses
- Caution in children
Interaction
- Possible additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or other CNS depressants
- Intensification of anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
- Potential risk of drug-induced involuntary movements during concomitant administration with other drugs
