Indications

  • Treatment of hypertension alone or with other antihypertensive agents
  • Lowering blood pressure
  • Initial therapy for patients likely to need multiple antihypertensive agents
  • Individualized decision for combination therapy based on baseline blood pressure and target goal
  • Variability in blood pressure goals based on patient's risk

Pharmacology

  • Amlodipine as a dihydropyridine calcium channel blocker
  • Greater effect on vascular smooth muscle cells than on cardiac muscle cells
  • Peripheral arterial vasodilator leading to reduction in peripheral vascular resistance and blood pressure
  • Olmesartan Medoxomil blocking vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Selective blocking of angiotensin II binding to AT1 receptor
  • Reversible, competitive inhibitor of the AT1 receptor
  • Does not inhibit ACE (kinase II)

Dosage & Administration

  • Substitution of individually titrated components
  • Increased amounts of Amlodipine, Olmesartan Medoxomil, or both as needed
  • Initial therapy with 5/20 mg once daily
  • Titration up to a maximum of 10/40 mg once daily
  • Recommended starting dose of Amlodipine as 2.5 mg in patients 75 years old or older

Interaction

  • Attenuation of antihypertensive effect by NSAIDs including selective COX-2 inhibitors
  • Close monitoring of blood pressure, renal function, and electrolytes in patients on combination therapy with other agents affecting the RAS

Contraindications

  • Cannot be co-administered with Aliskiren in patients with diabetes

  • Peripheral edema
  • Headache
  • Flushing
  • Dizziness
  • Sprue-like enteropathy

Pregnancy & Lactation

  • Pregnancy Category D
  • Contraindicated in 2nd and 3rd trimester
  • Potential adverse effects on nursing infant
  • Consideration of discontinuation of nursing or drug based on importance to the mother

Precautions & Warnings

  • Caution in volume- or salt-depleted patients for hypotension risk
  • Caution in patients with severe aortic stenosis for vasodilation risk
  • Caution in patients with severe obstructive coronary artery disease for angina or acute MI risk

Use in Special Populations

  • Safety and effectiveness not established in pediatric patients
  • No overall differences in safety or effectiveness between subjects 65 years of age or older and younger subjects
  • No studies in patients with renal impairment
  • Initial therapy not recommended in hepatically impaired patients

Overdose Effects

  • No information on over dosage in humans
