Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Formation of angiotensin II
- Vasoconstriction
- Stimulation of aldosterone synthesis and release
- Cardiac stimulation
- Renal reabsorption of sodium
- Blocking vasoconstrictor effects of angiotensin II
- Selective binding to AT 1 receptor
- No inhibition of ACE (kininase II)
- No effects on bradykinin response
- Inhibition of negative regulatory feedback of angiotensin II on renin secretion
Dosage & Administration
- Individualized dosage
- Usual recommended starting dose of 20 mg once daily
- Possible increase to 40 mg for further reduction in blood pressure
- No greater effect beyond 40 mg
- No advantage of twice-daily dosing over once daily
- No initial dosage adjustment for elderly patients
- No initial dosage adjustment for patients with moderate to marked renal impairment or hepatic dysfunction
- Consideration for lower starting dose in patients with possible depletion of intravascular volume
- May be administered with or without food
- Safety and effectiveness not established in pediatric patients
Interaction
- No significant drug interactions with digoxin or warfarin
- Bioavailability not significantly altered by antacids
- Not metabolized by cytochrome P450 system
- No effects on P450 enzymes
- Possible deterioration of renal function with NSAIDs including selective COX-2 inhibitors
- Attenuation of antihypertensive effect with NSAIDs including selective COX-2 inhibitors
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Well tolerated with adverse reactions similar to placebo
- Overall frequency of adverse reactions not dose related
- Common adverse reactions include dizziness, back pain, bronchitis, diarrhea, headache, hematuria, hyperglycemia, hypertriglyceridemia, influenza-like symptoms, pharyngitis, rhinitis, sinusitis
- Incidence of cough similar to placebo
Pregnancy & Lactation
- Discontinue product when pregnancy is detected
- Avoid use during second and third trimesters due to potential fetal injury and death
- Potential secretion in human milk with adverse effects on nursing infant
Precautions & Warnings
- Anticipated changes in renal function with treatment
- Association with oliguria, progressive azotemia, acute renal failure, and death in susceptible individuals
Overdose Effects
- No experience of overdose reported
- Likely effects include hypotension, tachycardia, and possibly bradycardia
- Consider gastric lavage or induction of emesis if intake is recent
- Clinically significant hypotension requires active cardiovascular system support
