Pharmacology
- Mode of action: Selective ß1 blocker, reduces heart rate, and cardiac output resulting in decreased arterial hypertension
- Absorption and bioavailability: Almost completely absorbed from the gastrointestinal tract with an absolute bioavailability of 88%
- Metabolism: Metabolized via oxidative pathways with no subsequent conjugation
- Elimination: Balanced between renal elimination and hepatic metabolism, with an elimination half-life of 10-12 hours
Dosage & Administration
- Adult: Usual starting dose of 5 mg once daily, which may be increased to 10 mg or 20 mg once daily if necessary
- Children: Safety and effectiveness not established
- Patients With Renal or Hepatic Impairment: Initial daily dose should be 5 mg
Interaction
- Other β-blocking Agents: Not recommended for combination with other β-blocking agents
- Catecholamine-Depleting Drugs: Patients receiving catecholamine-depleting drugs should be monitored closely
- Centrally Active Antihypertensive Agents: Caution required if coadministered with clonidine
- Antiarrhythmic Agents: Should be used with care when myocardial depressants or inhibitors of A-V conduction are used concurrently
Contraindications
Not recommended in patients with cardiogenic shock, overt heart failure, second or third-degree A-V block, right ventricular failure secondary to pulmonary hypertension, and sinus bradycardia

- Impaired renal or hepatic function: Use caution in adjusting the dose in patients with renal or hepatic impairment
- History of severe anaphylactic reaction: Patients may be more reactive to allergens
