Indications
- Treatment of invasive aspergillosis
- Treatment of candidemia and disseminated candidiasis
- Treatment of esophageal candidiasis
- Treatment of serious infections caused by specific fungal species incl. Scedosporium apiospermum and Fusarium Solani
- Intolerance or refractory to other therapy for fungal infections
Pharmacology
- Voriconazole is a triazole antifungal medication
- Inhibits ergosterol synthesis
- Binds and inhibits CYP450-dependent 14-alpha sterol demethylase
- Results in depletion of ergosterol in fungal cell membrane
Dosage & Administration
- Oral and Intravenous dosage recommendations based on body weight
- Loading dose 400mg or 10ml every 12 hours for adults at or over 40kg
- Maintenance dose 200mg or 5ml twice daily for adults at or over 40kg
- Specific dosage regimen for patients below 40kg body weight
- Different dosage regimen for invasive aspergillosis, candidemia, Scedosporiosis, and Fusariosis
Interaction
- Vefend dosage adjustment with CYP3A4, CYP2C9, and CYP2C19 inhibitors and inducers
- Possible increase in concentrations and activity of CYP3A4, CYP2C9, and CYP2C19 substrate drugs
- Increased maintenance oral and intravenous dosage with co-administration of Phenytoin or Efavirenz
Contraindications
- Hypersensitivity to Voriconazole or any other components
- Co-administration precautions with specific drugs due to risk of serious adverse reactions or loss of efficacy
Side Effects
- Common side effects include abdominal pain, anemia, blurred vision, headache, chest pain, nausea, and diarrhea
Pregnancy & Lactation
- Insufficient well-controlled studies in pregnant women
- Use during pregnancy only if potential benefit justifies potential risk to the fetus
Precautions & Warnings
- Long-term exposure assessment required for treatments exceeding 180 days
- Reports relating long-term treatment to squamous cell carcinoma of the skin (SCC)
