Indications
- Treatment of Rheumatoid Arthritis
- Inadequate response to TNF antagonist therapies
- Adult patients only
Pharmacology
- Selective and reversible JAK1 and JAK2 inhibitor
- Modulates signaling pathways
- Reduces phosphorylation and activation of STATs
Dosage & Administration
- Recommended dose of 2 mg once daily
- Monotherapy or in combination with Methotrexate or other DMARDs
- Oral administration with or without food
- Not established for pediatric use
- Dose adjustment for geriatric and hepatic impairment
- Not recommended for renal impairment (GFR < 60)
Interaction
- Increased exposure with strong OAT3 inhibitors
- Not studied in combination with other JAK inhibitors or biologic DMARDs
Contraindications
- Anemia (Hb < 8 g/dL)
- Lymphopenia (ALC < 500 cells/mm^3)
- Neutropenia (ANC < 1000 cells/mm^3)
Side Effects
- Common ADRs: increased LDL cholesterol, upper respiratory tract infections, nausea
Pregnancy & Lactation
- Contraindicated during pregnancy
- No information on presence in human milk
Precautions & Warnings
- Avoid use in patients with active, serious infection
- Not for patients with active TB
- Consider risks and benefits in patients with known malignancy
- Caution in patients at increased risk of thrombosis or gastrointestinal perforation
- To be dispensed only by prescription
