Indications

  • Treatment of panic disorder with or without agoraphobia
  • Adjunct in the treatment of Lennox-Gastaut Syndrome
  • Treatment of akinetic and myoclonic seizures
  • Indicated for patients with absence seizures who have failed succinimides therapy

Pharmacology

  • Exhibits anticonvulsive, sedative, muscle relaxing, and anxiolytic effects
  • Enhances GABAergic neurotransmission at inhibitory synapses
  • Shows effect on serotonin
  • Rapidly suppresses various types of paroxysmal activity

Dosage & Administration

  • Initial dose for adults with seizure disorders should not exceed 1.5 mg/day
  • Initial dose for adults with panic disorder is 0.25 mg
  • Pediatric dose based on body weight
  • Dosage may be increased in increments every 3 days
  • Different dosage for intravenous injection in infants, children, and adults

Interaction

  • Does not alter the pharmacokinetics of phenytoin, carbamazepine, or phenobarbital
  • Effect on the metabolism of other drugs has not been investigated

Contraindications

  • Hypersensitivity to benzodiazepines
  • Clinical or biochemical evidence of significant liver disease
  • Contraindicated in acute narrow angle glaucoma

Side Effects

  • CNS depression
  • Drowsiness in approximately 50% of patients
  • Ataxia in approximately 30% of patients
  • Behavior problems in approximately 25% of patients
  • Other side effects such as abnormal eye movements, coma, tremor, vertigo, confusion, depression, increased libido, insomnia, psychosis, palpitations

Pregnancy & Lactation

  • Potential teratogenic effects during pregnancy
  • Use only if benefits outweigh the risks
  • High doses in the last trimester can cause irregularities in the heartbeat of the unborn child
  • Breastfeeding should be discontinued if the drug is administered

Precautions & Warnings

  • May increase the incidence of generalized tonic-clonic seizures in patients with multiple seizure disorders
  • Concomitant use with valproic acid may produce absence status
  • Special considerations for pediatric, geriatric, renal impairment, and hepatic impairment use

Use in Special Populations

  • May cause increased production of saliva and bronchial secretion in infants and small children
  • Benzodiazepine pharmacologic effects appear to be greater in elderly patients
  • Renal impairment does not affect pharmacokinetics
  • Plasma protein binding in cirrhotic patients is significantly different

Overdose Effects

  • Common symptoms include drowsiness, ataxia, dysarthria, and nystagmus
  • May lead to areflexia, apnoea, hypotension, cardiorespiratory depression, and coma
  • Increased frequency of seizures at supratherapeutic plasma concentrations
  • Monitor vital signs and institute supportive measures
  • Prevent further absorption using appropriate methods
