Indications
- Treatment of acute and chronic psychoses

Pharmacology
- Belongs to dibenzothiazepine derivatives chemical class
- Antagonist at serotonin 5HT1A and 5HT2 receptors
- Antagonist at dopamine D1 and D2 receptors
- Antagonist at histamine H1 receptors
- Antagonist at adrenergic α1 and α2 receptors
- No appreciable affinity at cholinergic muscarinic and benzodiazepine receptors
- Mechanism of action is unknown
- Efficacy in schizophrenia is mediated through dopamine D2 and serotonin 5HT2 antagonism
- Antagonism of histamine H1 receptors may cause somnolence
- Antagonism of adrenergic α receptors may cause orthostatic hypotension
Dosage & Administration
- Administered twice daily
- First four days dosage titration
- Dosage range of 300-450 mg/day for acute and chronic psychoses
- Dosage range of 400-800 mg/day for manic episodes associated with bipolar disorder
- Usual dose of 300 mg/day for depressive episodes associated with bipolar disorder
- Total daily dose for the first four days of therapy
- Can be titrated based on clinical response and tolerability
- Maintenance therapy dosage adjustment based on clinical response
Interaction
- Caution with medicines causing electrolyte imbalance
- Caution with medicines increasing QT interval
- Co-administration with thioridazine or carbamazepine increases Quetiapine clearance
- Co-administration with phenytoin increases Quetiapine clearance
Contraindications
- Contra-indicated in patients hypersensitive to Quetiapine
Side Effects
- Common adverse drug reactions include somnolence, dizziness, dry mouth, withdrawal symptoms, elevated serum triglyceride levels, elevated total cholesterol, decreased HDL cholesterol, weight gain, decreased haemoglobin, extrapyramidal symptoms
Pregnancy & Lactation
- Safety and efficacy during human pregnancy not established
- Use during pregnancy only if benefits justify potential risks
- Administered dose and duration of treatment should be as low and short as possible
- Excretion into human milk is unknown
- Breast-feeding should be avoided while taking Quetiapine
Precautions & Warnings
- Increased risk of suicidal thoughts, self-harm and suicide with depression
- Caution in patients with known cardiovascular disease
- Caution in patients with cerebrovascular disease
- Caution in patients predisposed to hypotension
- Caution in patients with a history of seizures
- Consider dose reduction or discontinuation if signs of tardive dyskinesia appear
- Discontinue in case of neuroleptic malignant syndrome
- Exercise caution in patients with cardiovascular disease or family history of QT prolongation
- Severe neutropenia uncommonly reported
- Increases in blood glucose and hyperglycemia observed in clinical trials
Use in Special Populations
- Use with caution in the elderly
- Slower dose titration and lower daily therapeutic dose may be needed in the elderly
- Not indicated for use in children and adolescents below 18 years of age
Overdose Effects
- Survival reported in acute overdoses of up to 30 grams
- No specific antidote
- Consider multiple drug involvement in severe intoxication cases
- Intensive care procedures recommended
- Treatment of refractory hypotension with appropriate measures
