Indications
- Treatment of hypertension
- Can be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Selective AT 1 receptor blocker in vascular smooth muscle
- Does not inhibit ACE (kininase II)
- No effect on the response to bradykinin
Dosage & Administration
- Recommended starting dose of 20 mg once daily
- Dose may be increased to 40 mg for further reduction in blood pressure
- No advantage of twice-daily dosing over once daily
- No initial dosage adjustment for elderly patients or those with moderate to marked renal impairment
- Consideration for use of a lower starting dose in patients with possible depletion of intravascular volume
- May be administered with or without food
- Safety and effectiveness not established in pediatric patients
Drug Interaction
- No significant drug interactions reported with digoxin or warfarin
- Antacids do not significantly alter bioavailability
- Not metabolized by cytochrome P450 system
- No expected interactions with drugs that inhibit, induce, or are metabolized by P450 enzymes
- Co-administration with NSAIDs may result in deterioration of renal function
- Monitor renal function periodically in patients receiving Olmenor and NSAID therapy
- Attenuation of antihypertensive effect by NSAIDs
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Well tolerated with similar incidence of adverse reactions to placebo
- Overall frequency of adverse reactions not dose related
- Incidence of withdrawals due to adverse reactions at 2.4%
- Most common adverse reaction: dizziness (3%)
- Other adverse reactions include back pain, bronchitis, diarrhea, headache, hyperglycemia, sinusitis, cough, etc.
Pregnancy & Lactation
- Discontinue product when pregnancy is detected
- Drugs on renin-angiotensin system can cause harm to developing fetus
- Potential for adverse effects on nursing infant
- Decision should be made whether to discontinue nursing or drug, considering the importance of the drug to the mother
Precautions & Warnings
- Changes in renal function may be anticipated in susceptible individuals
- Associated with oliguria, progressive azotemia, acute renal failure, and death in patients whose renal function depends on renin-angiotensin-aldosterone system
Overdose Effects
- Likely effects: hypotension, tachycardia, bradycardia if parasympathetic stimulation occurs
- Consider gastric lavage or induction of emesis if intake is recent
- Clinically significant hypotension due to overdose requires active support of cardiovascular system
