Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Inhibits negative regulatory feedback on renin secretion
Dosage & Administration
- Usual starting dose of 20 mg once daily
- May increase to 40 mg if further reduction in blood pressure is required
- No advantage in twice-daily dosing over once daily
- No initial dosage adjustment recommended for elderly patients or those with renal or hepatic impairment
- Consideration for lower starting dose in patients with possible intravascular volume depletion
- May be administered with or without food
- Safety and effectiveness in pediatric patients not established
Interaction
- No significant drug interactions reported with digoxin or warfarin
- Bioavailability not significantly altered by antacids
- Not metabolized by cytochrome P450 system
- Potential renal function deterioration with NSAID co-administration
- Antihypertensive effect may be attenuated by NSAIDs
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Well tolerated with similar adverse reaction incidence to placebo
- Main adverse reaction was dizziness
- Other adverse reactions occurred at similar or greater incidence in patients receiving placebo
Pregnancy & Lactation
- Discontinue product when pregnancy is detected
- Can cause injury to developing fetus when used during second and third trimesters
- Potential secretion in human milk
- Decision should be made whether to discontinue nursing or drug
Precautions & Warnings
- Anticipated changes in renal function due to inhibiting the renin-angiotensin system
- Oliguria, progressive azotemia, and acute renal failure may occur in susceptible individuals
Overdose Effects
- Experience of overdose not reported
- Likely effects include hypotension and tachycardia
- Bradycardia could occur with parasympathetic stimulation
- Actions such as gastric lavage or emesis induction may be considered for recent intake
