Indications
- Reduce rate of myocardial infarction (MI)
- Reduce rate of stroke
- Non-ST-segment elevation ACS
- ST-elevation myocardial infarction (STEMI)
- Established peripheral arterial disease
- Recent myocardial infarction
- Recent stroke
- Unstable angina (UA)
- Non-ST-elevation myocardial infarction (NSTEMI)
Pharmacology
- Prodrug
- Inhibits platelet activation
- Irreversible binding to P2Y12 class of ADP receptors
- Dose-dependent inhibition of platelet aggregation
Dosage & Administration
- Oral loading dose of 300 mg
- Maintenance dose of 75 mg once daily
- With or without food
Interaction
- Increased risk of bleeding with NSAIDs, warfarin, SSRIs, SNRIs
- Avoid concomitant use with CYP2C19 inhibitors and Repaglinide (CYP2C8 substrates)
Contraindications
- Hypersensitivity to the drug substance
- Active pathological bleeding
Side Effects
- Common: Bleeding, Diarrhoea, Gastrointestinal discomfort, Haemorrhage, Skin reactions
- Rare: Acquired haemophilia, Anaemia, Angioedema, Arthralgia, Bone marrow disorders
Pregnancy & Lactation
- Use during pregnancy only if clearly needed
- Unknown excretion in human breast milk
Precautions & Warnings
- Metabolism impairment by genetic variations in CYP2C19
- Inhibition by Omeprazole and Esomeprazole
- Increased risk of bleeding
- Discontinuation risk of cardiovascular events
- Thrombotic Thrombocytopenic Purpura (TTP) reported
- Hypersensitivity reactions reported
- Resume after major bleeding risk surgery
Use in Special Populations
- Safety and effectiveness not established in pediatric populations
- No dosage adjustment necessary in elderly patients
Overdose Effects
- May lead to bleeding complications
- Platelet transfusion may restore clotting ability
