Indications
- Treatment of erectile dysfunction in men
- Treatment of pulmonary arterial hypertension
Description
- Selective, reversible inhibitor of phosphodiesterase type 5 (PDE5)
- Increases levels of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum
Mechanism of Action
- Involves release of nitric oxide in the corpus cavernosum
- Activates the enzyme guanylate cyclase
- Results in smooth muscle relaxation and inflow of blood into penile tissues
Pharmacokinetics and Metabolism
- Mean absolute bioavailability of 41%
- Eliminated predominantly by hepatic metabolism
- Terminal half lives of about 4 hours
Dosage & Administration
- Recommended starting dose of 50 mg
- Dose may be increased to a maximum of 100 mg or decreased to 25 mg
- Maximum recommended dosing frequency is once per day
Interaction
- Metabolism mediated by cytochrome P450 (CYP) isoforms 3A4 and 2C9
- Inhibitors reduce clearance and inducers increase clearance
- Inhibitors like cimetidine and erythromycin increase systemic exposure of Sildenafil
- Strong CYP3A4 inhibitors expected to have greater effects
Contraindications
- Hypersensitivity to any component of the medication
- Concomitant use of organic nitrates
Side Effects
- Body as a whole: face edema, chills, abdominal pain
- Cardiovascular: angina pectoris, palpitation, hypotension
- Digestive: vomiting, dysphagia, liver function tests abnormal
- Metabolic and Nutritional: gout, hyperglycemia, hypoglycemic reaction
- Respiratory: asthma, dyspnea, bronchitis
- Skin and Appendages: urticaria, pruritus, exfoliative dermatitis
- Special Senses: sudden decrease or loss of hearing, mydriasis, photophobia
Pregnancy & Lactation
- Pregnancy category B
- No well-controlled studies in pregnant women
- Not indicated for use by women
Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- Potential for additive effect on blood pressure
- Consideration required for patients with bleeding disorders and active peptic ulceration
- Caution in patients with anatomical deformation of the penis or conditions predisposing to priapism
Overdose Effects
- Adverse events at higher incidence rates and severities
- Standard supportive measures should be adopted in cases of overdose
- Renal dialysis not expected to accelerate clearance
