Indications
- Acute Coronary Syndrome (ACS) - Reduce rate of myocardial infarction (MI) and stroke
- Recent MI, recent Stroke, or established Peripheral Arterial Disease - Reduce rate of MI and stroke
Pharmacology
Clopidogrel is a prodrug that inhibits platelet activation and aggregation through irreversible binding of its active metabolite to the P2Y12 class of ADP receptors on platelets.
Dosage & Administration
- Acute Coronary Syndrome - 300 mg oral loading dose, then 75 mg once daily
- Recent MI, Recent Stroke, or Established Peripheral Arterial Disease - 75 mg once daily orally
Interaction
- NSAIDs, warfarin, SSRIs, SNRIs - Increases risk of bleeding
- CYP2C19 inhibitors (omeprazole or esomeprazole) - Avoid concomitant use
- Repaglinide (CYP2C8 substrates) - Avoid concomitant use as it increases plasma concentrations of Repaglinide
Contraindications
Hypersensitivity to the drug substance or any component, active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Common: Bleeding, Diarrhoea, gastrointestinal discomfort, haemorrhage, Skin reactions
- Rare: Acquired haemophilia, anaemia, angioedema, arthralgia, arthritis, bone marrow disorders
Pregnancy & Lactation
No adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Unknown excretion in human breast milk.
Precautions & Warnings
- Metabolism impaired by genetic variations in CYP2C19 or drugs that inhibit CYP2C19
- Inhibits platelet aggregation for lifetime of the platelet (7-10 days), increases bleeding risk
- Discontinuation increases risk of cardiovascular events, discontinue 5 days prior to elective surgery with major risk of bleeding
Use in Special Populations
Safety and effectiveness in pediatric populations have not been established. No dosage adjustment necessary in elderly patients.
Overdose Effects
May lead to bleeding complications, platelet transfusion may restore clotting ability.
