Indications
- treatment for primary biliary cholangitis (PBC)
- combination therapy with ursodeoxycholic acid (UDCA)
- monotherapy for patients unable to tolerate UDCA
Pharmacology
- agonist for Farnesoid X Receptor (FXR)
- regulator of bile acid, inflammatory, and metabolic pathways
- decreases intracellular hepatocyte concentrations of bile acids
Mechanism of Action
- suppresses de novo synthesis of bile acids from cholesterol
- increases transport of bile acids out of hepatocytes
- limits the overall size of circulating bile acid pool
- promotes choleresis
Pharmacodynamics
- associated with a 173% increase in concentrations of FGF-19
- reduces concentrations of cholic acid and chenodeoxycholic acid
Dosage & Administration
- initial dosage of 5 mg once daily for 3 months
- maximum dosage of 10 mg once daily
- monitoring for biochemical response and tolerability
- management of intolerable pruritus

- interaction with bile acid binding resins
- interaction with warfarin
- potential interaction with CYP1A2 substrates

Contraindications
- decompensated cirrhosis
- compensated cirrhosis with evidence of portal hypertension
- complete biliary obstruction
