Indications
- Post-menopausal osteoporosis
- Renal osteodystrophy
- Postsurgical hypoparathyroidism
- Idiopathic hypoparathyroidism
- Pseudohypoparathyroidism
- Secondary hyperparathyroidism in patients with moderate to severe chronic renal failure (pre-dialysis)
- Vitamin D dependent rickets
Pharmacology
Calcitriol promotes intestinal absorption of calcium and regulates bone mineralization
Dosage & Administration
- 0.25 mcg twice daily for post-menopausal osteoporosis
- 0.25 mcg every other day for renal osteodystrophy (dialysis patients)
- 0.25 mcg/day for hypoparathyroidism and rickets
- 1-2 mcg administered three times weekly for secondary hyperparathyroidism
- 0.02 mcg/kg for initial intravenous dose of Calcitriol injection
Interaction
Concomitant treatment with thiazide diuretics increases the risk of hypercalcemia
Contraindications
Hypersensitivity to any of its ingredients, and all diseases associated with hypercalcemia

Pregnancy & Lactation
May be used during pregnancy only if benefits outweigh potential risk to the fetus; serum calcium levels of mother and infant should be monitored during breastfeeding
Precautions & Warnings
- Excessive dosage induces hypercalcemia and hypercalciuria
- Given cautiously to patients on digitalis
- Discontinuation may result in rebound effect
- Vitamin D and its derivatives should be withheld during treatment
- A non-aluminum phosphate-binding compound should be used to control serum phosphorus levels in patients undergoing dialysis
- Chronic hypercalcemia can lead to generalized vascular calcification
Use in Special Populations
- Recommended initial dosage of 0.25 mcg/day in adults and pediatric patients 3 years of age or older
- Dosage may be increased if necessary to 0.5 mcg/day
- Recommended initial dosage of Caloren is 10 to 15 ng/kg/day for pediatric patients less than 3 years of age
Overdose Effects
Can cause hypercalcemia, hypercalciuria, and hyperphosphatemia
