Indications
- Reduce rate of myocardial infarction (MI)
- Reduce rate of stroke
- Treatment for acute coronary syndrome (ACS)
- Treatment for unstable angina (UA)
- Treatment for non-ST-elevation myocardial infarction (NSTEMI)
- Reduce rate of MI in patients with STEMI
- Treatment for recent MI
- Treatment for recent stroke
- Treatment for established peripheral arterial disease
Pharmacology
- Clopidogrel is a prodrug
- Inhibits platelet activation
- Inhibits platelet aggregation
- Binds to P2Y12 class of ADP receptors
- Dose-dependent inhibition of platelet aggregation
- Inhibition visible 2 hours after single oral doses
Dosage & Administration
- Initial loading dose of 300 mg
- Maintenance dose of 75 mg once daily
- Can be taken with or without food
- Different dosages for acute coronary syndrome and other conditions
Interactions
- Increases risk of bleeding with NSAIDs, warfarin, SSRIs, SNRIs
- Avoid concomitant use with omeprazole or esomeprazole
- Interaction with Repaglinide leads to increased plasma concentrations
Contraindications
- Hypersensitivity to clopidogrel
- Active pathological bleeding
- Peptic ulcer
- Intracranial hemorrhage
Side Effects
- Common: Bleeding, Diarrhoea, Gastrointestinal discomfort, Skin reactions
- Rare: Acquired haemophilia, Anaemia, Arthralgia, Bone marrow disorders
Pregnancy & Lactation
- Use during pregnancy only if clearly needed
- Unknown excretion in human breast milk
- Decision on drug discontinuation while nursing

- Genetic variations affecting metabolism
- Inhibition by drugs like Omeprazole and Esomeprazole
- Life-long platelet inhibition
- Increased risk of bleeding
- Risk of cardiovascular events upon discontinuation
- TTP reported with urgent treatment
- Hypersensitivity reactions reported
