Description
- Clopidogrel tablets are indicated for reducing the rate of myocardial infarction (MI) and stroke in patients with acute coronary syndrome (ACS), recent MI, recent stroke, or established peripheral arterial disease.
Indications
- Reduction of MI and stroke in non-ST-segment elevation ACS
- Reduction of MI and stroke in acute ST-elevation myocardial infarction (STEMI)
- Reduction of MI and stroke in patients with established peripheral arterial disease or with a history of recent MI or recent stroke
Pharmacology
- Inhibition of platelet activation and aggregation
- Irreversible binding to P2Y12 class of ADP receptors on platelets
Dosage & Administration
- 300 mg oral loading dose for patients needing an immediate antiplatelet effect
- 75 mg once daily orally without a loading dose
- Oral administration with or without food
Interactions
- Increased risk of bleeding with NSAIDs, warfarin, selective serotonin and serotonin norepinephrine reuptake inhibitors (SSRIs, SNRIs)
- Avoid concomitant use with CYP2C19 inhibitors such as omeprazole or esomeprazole
- Increased plasma concentrations of Repaglinide with concomitant use of CYP2C8 substrates
Contraindications
- Hypersensitivity to the drug or any component
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Common: Bleeding, diarrhea, gastrointestinal discomfort, skin reactions
- Rare: Acquired haemophilia, anemia, angioedema, arthralgia, bone marrow disorders
Pregnancy & Lactation
- Limited data in pregnant women
- Unknown excretion in human breast milk
- Use during pregnancy only if clearly needed
Precautions & Warnings
- Impaired metabolism in CYP2C19 poor metabolizers and with drugs that inhibit CYP2C19
- Increased risk of bleeding due to lifetime inhibition of platelet aggregation
- Risk of discontinuation leading to cardiovascular events
- Reports of Thrombotic Thrombocytopenic Purpura (TTP), hypersensitivity reactions
Use in Special Populations
- Safety and effectiveness not established in pediatric populations
- No dosage adjustment necessary in elderly patients
Overdose Effects
- May lead to bleeding complications
- Potential restoration of clotting ability with platelet transfusion
