Indications
- Treatment of Rheumatoid Arthritis
- Inadequate response to tumor necrosis factor (TNF) antagonist therapies
Pharmacology
- Selective and reversible inhibitor of Janus kinase JAK1 and JAK2
- Transduction of intracellular signals
- Modulation of signaling pathways
- Reduction of phosphorylation and activation of STATs
Dosage & Administration
- Recommended dose of 2 mg once daily
- Use as monotherapy or in combination with Methotrexate or other DMARDS
- Oral administration with or without food
- Not established for pediatric use
- Careful dose selection for geriatric use
- No dose adjustment for mild or moderate hepatic impairment
- Not recommended in renal impairment with estimated GFR less than 60 mL/min/I .73 m2
Interaction
- Increased exposure when co-administered with strong OAT3 inhibitors
- Not studied in combination with other JAK inhibitors or biologic DMARDS
Contraindications
- Avoid initiation or interrupt in anemia (hemoglobin less than 8 g/dL)
- Avoid initiation or interrupt in lymphopenia (Absolute Lymphocyte Count less than 500 cells/mm^3)
- Avoid initiation or interrupt in neutropenia (Absolute Neutrophil Count less than 1000 cells/mm^3)
Side Effects
- Common adverse drug reactions: increased LDL cholesterol, upper respiratory tract infections, nausea
Pregnancy & Lactation
- Contraindicated during pregnancy
- No information on presence in human milk
Precautions & Warnings
- Avoid use in patients with active, serious infections
- Not to be given to patients with active TB
- Consider risks and benefits in patients with known malignancy
- Use with caution in patients at risk of thrombosis
- Use with caution in patients at risk for gastrointestinal perforation
- To be dispensed only by the prescription of a registered physician

- Monitoring for signs and symptoms of adverse reactions
