Indications
- Reduction of myocardial infarction rate in ACS patients
- Reduction of stroke rate in ACS patients
- Reduction of myocardial infarction and stroke in STEMI patients
- Reduction of MI and stroke in patients with peripheral arterial disease
- Reduction of MI and stroke in patients with recent MI or stroke
Pharmacology
Irreversible binding of active metabolite to P2Y12 class of ADP receptors on platelets
Dosage & Administration
- 300 mg oral loading dose for acute coronary syndrome
- 75 mg once daily orally for recent MI, stroke, or peripheral arterial disease
- Oral administration with or without food
Interaction
- Increased bleeding risk with NSAIDs, warfarin, SSRIs, SNRIs
- Avoid concomitant use with CYP2C19 inhibitors
- Increased plasma concentrations of Repaglinide with concomitant use
Contraindications
- Hypersensitivity to drug substance or components
- Active pathological bleeding
Side Effects
- Common: Bleeding, diarrhea, gastrointestinal discomfort, hemorrhage, skin reactions
- Rare: Acquired hemophilia, anemia, angioedema, arthralgia, bone marrow disorders
Pregnancy & Lactation
Use during pregnancy only if clearly needed, unknown excretion in human breast milk
Precautions & Warnings
- Impaired metabolism in CYP2C19 poor metabolizers
- Increased bleeding risk due to lifetime platelet inhibition
- Discontinuation increases risk of cardiovascular events
- Reported cases of TTP, hypersensitivity reactions
Use in Special Populations
Safety and effectiveness not established in pediatric populations, no dosage adjustment in elderly patients
Overdose Effects
May lead to bleeding complications, platelet transfusion may restore clotting ability
