Description
- Accuzith is indicated for infections in lower respiratory tract, upper respiratory tract, otitis media, and skin and soft tissue infections. It is also indicated in the treatment of non-gonococcal urethritis and cervicitis due to Chlamydia trachomatis. Azithromycin, the active ingredient, is acid-stable and can be taken orally without protection from gastric acids. It has high concentrations in phagocytes, facilitating transport to the site of infection. Azithromycin acts by binding to the 50S ribosomal subunit of susceptible microorganisms, interfering with microbial protein synthesis without affecting nucleic acid synthesis.
Dosage
- Oral Adult: 500 mg once daily for 3 days or 500 mg on day 1, then 250 mg daily for 4 days. For sexually transmitted diseases, the dose is 1 gm as a single dose or 500 mg on day 1, followed by 250 mg daily for 2 days. Children: 10 mg/kg body weight once daily for 3 days for children over 6 months.
Administration
- Reconstitution procedure of suspension: Shake well, add water up to the mark, and shake until powder is completely mixed with water.
Interaction
- Azithromycin and antacids should be taken at least 1 hour apart. Caution should be exercised when co-administering azithromycin and cyclosporin, digoxin, ergot derivatives, methylprednisolone, theophylline, warfarin, and terfenadine. Azithromycin had no significant effect on the plasma levels of carbamazepine.
Contraindications
- Hypersensitivity to azithromycin or any other macrolide antibiotic. Co-administration of ergot derivatives and azithromycin. Patients with hepatic diseases.
Side Effects
- Gastrointestinal side-effects (nausea, vomiting, diarrhoea, abdominal discomfort), allergic reactions, reversible elevations in liver transaminases, cholestatic jaundice, transient mild reductions in neutrophil counts, reversible hearing impairment with prolonged higher doses.
Pregnancy & Lactation
- Pregnancy Category B. Caution advised during pregnancy and lactation, with use only if adequate alternatives are not available.
Precautions & Warnings
- Rare serious allergic reactions reported. Caution in renal impairment, avoidance in severe hepatic impairment.
Overdose Effects
- Typical symptoms include hearing loss, severe nausea, vomiting, and diarrhea. Gastric lavage and supportive measures are indicated.

- Keep in a dry place away from light and heat, out of the reach of children.

Use in Renal Impairment
- No dose adjustment needed in patients with mild renal impairment; caution in more severe renal impairment.
Use in Hepatic Impairment
- Avoid use in patients with hepatic disease as the liver is the principal route of excretion of azithromycin.

- No evidence to suggest an effect on a patient’s ability to drive or operate machinery.
