Indications

  • Treatment of hypertension
  • Treatment of hypertension with other antihypertensive agents
  • Initial therapy for patients needing multiple drugs to achieve blood pressure goals

Pharmacology

  • Fixed dose combination of Telmisartan and Amiodipine
  • Telmisartan blocks vasoconstrictor and aldosterone-secreting effects
  • Amiodipine inhibits transmembrane influx of calcium ion into vascular smooth muscle and cardiac muscle

Dosage & Administration

  • Usual starting dose is 40/5 mg once daily
  • Patients requiring larger blood pressure reductions may start with 80/5 mg once daily
  • Dosage must be individualized and may be increased after at least 2 weeks

Interactions

  • Co-administration of telmisartan did not result in major interaction with various medications
  • Amiodipine has been safely administered with several other medications

Contraindications

  • Hypersensitivity to the product or its components
  • Pregnancy & lactation
  • Biliary obstructive disorders, severe hepatic impairment, hypotension, cardiogenic shock, left ventricle outflow tract obstruction

Precautions & Warnings

  • Avoid fetal or neonatal exposure
  • Correct any volume or salt depletion before initiating therapy
  • Monitor for worsening heart failure
  • Avoid concomitant use of ACE inhibitor and angiotensin receptor blocker

Use in Special Populations

  • Safety and effectiveness in pediatric patients not established
  • Initial therapy not recommended for patients ≥75 years old or with hepatic impairment

Overdose Effects

  • Manifestations of overdose with telmisartan: hypotension, dizziness, tachycardia
  • Manifestations of overdose with amiodipine: excessive peripheral vasodilation, marked hypotension
