Pharmacology
- Potent COX-2 specific inhibitor
- Inhibition of COX-2 decreases clinical signs and symptoms
- Decreased potential for GI toxicity
- Dose-dependent COX-2 inhibition without COX-1 inhibition
Dosage & Administration
- 30 mg once daily for Osteoarthritis
- 90 mg once daily for Rheumatoid arthritis and Ankylosing spondylitis
- 120 mg once daily for Acute gouty arthritis
- 90 mg once daily for Postoperative dental surgery pain
- Shortest duration and lowest effective daily dose recommended
Interaction
- Increase in prothrombin time with oral anticoagulants
- Reduction in effect of diuretics and antihypertensive drugs with NSAIDs
- Concomitant use with low-dose acetylsalicylic acid for cardiovascular prophylaxis
- Potential increase in nephrotoxic effect with ciclosporin or tacrolimus
- Decrease in lithium renal excretion with NSAIDs
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or active GI bleeding
- Bronchospasm, acute rhinitis, nasal polyps, allergic-type reactions after taking acetylsalicylic acid or NSAIDs
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease
- Congestive heart failure
- Uncontrolled hypertension
- Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Precautions & Warnings
- Caution in patients at risk of GI complications with NSAIDs
- Consideration for patients with significant cardiovascular risk factors
- Monitoring of renal function in patients with reduced prostaglandin formation
- Caution in patients with history of cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring for liver dysfunction and signs of hypersensitivity
- Caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- No significant toxicity up to 500 mg single dose
- Supportive measures in case of overdose
