Pharmacology

  • Inhibitor of gastric acid secretion
  • Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
  • Onset of antisecretory effect within one hour
  • Maximum effect occurring within two hours
  • Inhibition of secretion lasts up to 72 hours
  • After discontinuation, secretory activity returns gradually over 3 to 5 days

Dosage

  • 20 mg once daily for benign gastric and duodenal ulcer
  • 20 mg once daily for NSAID-associated duodenal or gastric ulcer
  • 20 mg once daily for gastro-esophageal reflux disease
  • 10-20 mg daily for long-term management of acid reflux disease
  • 10-20 mg once daily for acid-related dyspepsia
  • 40 mg once daily for prophylaxis of acid aspiration
  • 60 mg once daily for Zollinger-Ellison syndrome
  • Omeprazole is recommended at a dose of 20 mg twice daily for Helicobacter pylori eradication regimen in peptic ulcer disease
  • Oral and IV injection options

Administration

  • IV injection for intravenous administration only
  • IV injection should be given as a slow intravenous injection
  • IV infusion should be given as an intravenous infusion over 20-30 minutes or more
  • Reconstituted solution should not be mixed or co-administered with any other drug
  • Directions for use of IV Injection and IV Infusion

Interaction

  • Decreased intragastric acidity affecting the absorption of ketoconazole
  • Metabolized in the liver through cytochrome P450
  • Can delay the elimination of diazepam, phenytoin, and warfarin
  • Monitoring of patients receiving warfarin or phenytoin is recommended
  • Increased plasma concentrations during concomitant administration with certain drugs
  • No evidence of interaction with alcohol, food, certain NSAIDs, and other specific drugs

Contraindications

  • Known hypersensitivity to omeprazole
  • Need to exclude the possibility of malignancy when gastric ulcer is suspected

Side Effects

  • Mild and reversible adverse reactions
  • Skin rash, urticaria, and pruritus
  • Photosensitivity, bullous eruption, and erythema multiforme
  • Diarrhoea, headache, and gastrointestinal reactions
  • Dry mouth, stomatitis, and candidiasis
  • Paraesthesia, dizziness, and feeling faint
  • Somnolence, insomnia, and vertigo
  • Reversible mental confusion, agitation, and depression
  • Arthritic and myalgic symptoms
  • Other isolated cases of adverse reactions

Pregnancy & Lactation

  • No adverse effects of omeprazole on pregnancy or the health of the fetus/newborn child
  • Use during pregnancy is considered safe
  • No information available on the passage into breast milk or effects on the neonate
  • Use of omeprazole during breastfeeding should be discontinued if considered essential
  • Safety and effectiveness not established in pediatric patients less than 18 years of age

Precautions & Warnings

  • Avoid concomitant use of clopidogrel and omeprazole
  • Possible association with increased risk for osteoporosis-related fractures
  • Atrophic gastritis noted occasionally in patients treated long-term with omeprazole
  • Concomitant use with methotrexate may lead to methotrexate toxicities
