Pharmacology

  • Triazole antifungal agent
  • Potent inhibitor of fungal cytochrome P-450 dependent enzymes
  • Responsible for the synthesis of ergosterol
  • Essential component of fungal cell membrane

Dosage administration

  • Dosage for Vaginal Candidiasis: 150 mg as a single dose for adults
  • Dosage for Oropharyngeal Candidiasis: 200 mg in 1st day followed by 100 mg daily for 14 days for adults
  • Dosage for Oesophageal Candidiasis: 200 mg in 1st day followed by 100 mg daily for 14-30 days for adults
  • Dosage for Tinea corporis/Tinea cruris/Tinea pedis/Other Tinea: 150 mg weekly for 4-6 weeks for adults
  • Dosage for Kerion: 50 mg daily for 20 days for adults
  • Dosage for Pityriasis versicolor: 400 mg as a single dose for adults
  • Dosage for Onychomycosis: 150 mg weekly for 12 months for adults
  • Dosage for Invasive candidal infections and cryptococcal infections: Orally or by IV infusion, 400 mg on first day then 200-400 mg daily for adults
  • Dosage for Prevention of cryptococcal meningitis: Orally or by IV infusion 200 mg daily for adults
  • Dosage for Prevention of fungal infections in immunocompromised patients: Orally or by IV infusion, 50-400 mg daily for adults
  • Dosage for children over 1 year: 1-2 mg/kg daily for Superficial Candidiasis
  • Dosage for children over 1 year: 3-6 mg/kg daily for Systemic Candidiasis & Cryptococcal infection

Interaction

  • Increased prothrombin time after warfarin administration
  • Prolonged serum half-life of concomitantly administered oral sulphonylureas
  • Effect of co-administration of hydrochlorothiazide to healthy volunteers
  • Increased level of phenytoin with co-administration of fluconazole
  • Decrease in AUC and shorter half-life with co-administration of rifampicin
  • No relevant effects on hormone level with combined oral contraceptives
  • Slowly increased cyclosporin concentrations in renal transplant patients
  • No clinically significant impairment of fluconazole absorption with food, cemetidine, antacids

Contraindications

  • Known hypersensitivity to Fluconazole or to related triazole compounds

Side effects

  • Nausea
  • Abdominal discomfort
  • Diarrhoea
  • Flatulence
  • Rash (Incidence less than 1%)
  • Anaphylaxis (rare)

Pregnancy lactation

  • Adverse fetal effects seen in animals at high dose levels
  • Not recommended in pregnancy or in women of childbearing potential
  • Similar concentrations in human breast milk as in plasma

Precautions warnings

  • Observation of abnormalities in hepatic, renal, haematological and other biochemical function tests in some patients
  • Rarely observed exfoliative cutaneous reactions
  • Discontinue therapy if rash develops in patients treated for superficial fungal infection
  • Not recommended for use during lactation
  • Unlikely to impair ability to drive or use machinery

Use in special populations

  • Normal dose for use in the elderly if no evidence of renal impairment
  • Dosage interval or daily dosage modification in patients with renal impairment
  • Excreted predominantly in the urine as unchanged drug

Overdose effects

  • Supportive measures and symptomatic treatment with gastric lavage if necessary
  • Excreted largely in the urine
  • Forced volume diuresis may increase the elimination rate
  • Three-hour session of hemodialysis decreases plasma levels by approximately 50%
