Indications
- Reduce rate of myocardial infarction (MI)
- Reduce rate of stroke
- Acute Coronary Syndrome (ACS)
- Unstable angina (UA)
- Non-ST-elevation myocardial infarction (NSTEMI)
- Recent MI
- Recent Stroke
- Established Peripheral Arterial Disease
Pharmacology
- Prodrug
- Inhibits platelet activation and aggregation
- Irreversible binding to P2Y12 class of ADP receptors
- Dose-dependent inhibition of platelet aggregation
Dosage & Administration
- 300 mg oral loading dose for acute coronary syndrome
- 75 mg once daily orally
- Oral administration with or without food
Interaction
- Increases risk of bleeding with NSAIDs, warfarin, SSRIs, and SNRIs
- Avoid concomitant use with CYP2C19 inhibitors like omeprazole or esomeprazole
- Increases plasma concentrations of Repaglinide when used with Repaglinide
Contraindications
- Hypersensitivity to the drug substance
- Active pathological bleeding like peptic ulcer or intracranial hemorrhage
Side Effects
- Common: Bleeding, Diarrhoea, gastrointestinal discomfort, haemorrhage, Skin reactions
- Rare: Acquired haemophilia, anaemia, angioedema, arthralgia, arthritis, bone marrow disorders
Pregnancy & Lactation
- Use only if clearly needed during pregnancy
- Unknown excretion in breast milk
- Consideration for discontinuing nursing or drug based on importance to the mother
Precautions & Warnings
- Metabolism impairment by genetic variations in CYP2C19 and drugs that inhibit CYP2C19
- Inhibits platelet aggregation for 7-10 days
- Increased risk of bleeding
- Discontinuation increases risk of cardiovascular events
- Reports of Thrombotic Thrombocytopenic Purpura (TTP)
- Reports of hypersensitivity reactions
Use in Special Populations
- Safety and effectiveness not established in pediatric populations
- No dosage adjustment necessary in elderly patients
Overdose Effects
- May lead to bleeding complications
- Platelet transfusion may restore clotting ability
