Indications
- Reduce the rate of myocardial infarction in patients with non-ST-segment elevation ACS
- Reduce the rate of stroke in patients with non-ST-segment elevation ACS

- Reduce the rate of stroke in patients with acute ST-elevation myocardial infarction

Pharmacology
- Prodrug
- Inhibits platelet activation and aggregation through irreversible binding
- Dose-dependent inhibition of platelet aggregation after single oral doses
- Inhibition reaches steady state between Day 3 and Day 7
Dosage & Administration
- 300 mg oral loading dose for acute coronary syndrome
- Continuation at 75 mg once daily for acute coronary syndrome
- 75 mg once daily without a loading dose for recent MI, recent Stroke, or established Peripheral Arterial Disease
- Oral administration with or without food
Interaction
- Increases risk of bleeding when used with NSAIDs, warfarin, SSRIs, SNRIs
- Avoid concomitant use with CYP2C19 inhibitors (omeprazole or esomeprazole)
- Avoid concomitant use with Repaglinide as it increases plasma concentrations of Repaglinide
Contraindications
- Hypersensitivity to the drug substance or any component of the product
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Common side effects: Bleeding, Diarrhoea, gastrointestinal discomfort, haemorrhage, Skin reactions
- Rare side effects: Acquired haemophilia, anaemia, angioedema, arthralgia, arthritis, bone marrow disorders
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Use during pregnancy only if clearly needed
- Unknown excretion in human breast milk
- Decision making: continuing nursing or discontinuing the drug
Precautions & Warnings
- Metabolism impairment due to genetic variations in CYP2C19
- Increased risk of bleeding due to lifelong platelet aggregation inhibition
- Discontinuation increases the risk of cardiovascular events
- Reported cases of Thrombotic Thrombocytopenic Purpura (TTP)
- Hypersensitivity reactions reported
Use in Special Populations
- Safety and effectiveness in pediatric populations not established
- No dosage adjustment necessary in elderly patients

- Platelet transfusion may restore clotting ability
