Description

  • A fixed-dose combination of Amlodipine and Atenolol for essential hypertension and angina pectoris

Dosage

  • Recommended dosage of Amlodipine and Atenolol 5/25 mg tablet once daily
  • Dosage may be increased to 5/25 mg two tablets daily

Indications

  • Indicated for patients with essential hypertension
  • Indicated for patients with angina pectoris & hypertension as co-existing diseases
  • Indicated for post myocardial infarction patients
  • Indicated for patients with refractory angina pectoris where nitrate therapy has failed

Pharmacology

  • Amlodipine is a dihydropyridine calcium antagonist that inhibits transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle
  • Atenolol is a cardioselective beta-blocker that reduces blood pressure by lowering cardiac output and decreasing sympathetic outflow from CNS

Interaction

  • Interaction with Disopyramide leading to additive negative inotropic effects on the heart
  • Interaction with Ampicillin at doses of 1 gm and above may reduce Atenolol levels
  • Interaction with oral antidiabetics and insulin leading to decreased tissue sensitivity to insulin and inhibited insulin secretion

Contraindications

  • Hypersensitivity to either component
  • Sinus bradycardia
  • Second and higher degrees of heart block
  • Cardiogenic shock
  • Hypotension
  • Congestive heart failure
  • Poor left ventricular function

  • Overall side-effects include fatigue
  • Overall side-effects include headache
  • Overall side-effects include edema
  • Overall side-effects include nausea
  • Overall side-effects include drowsiness
  • Overall side-effects include anxiety
  • Overall side-effects include depression

Pregnancy lactation

  • Use during pregnancy if expected benefit outweighs potential fetal risk
  • Not recommended for nursing mothers

Precautions warnings

  • Use with caution in patients with airway obstruction
  • Can be used in patients with renal impairment, caution if creatinine clearance is less than 30 ml/min
  • Caution in use for patients with severe liver damage due to prolongation of the elimination half-life of Amlodipine
  • Patients should be warned against stopping the drug suddenly, any discontinuation should be gradual and under observation

Overdose effects

  • Hypotension and less frequently congestive cardiac failure may occur in cases of overdosage
  • Unabsorbed drugs may be removed by gastric lavage or administration of activated charcoal
  • Symptomatic treatment is suggested
