Indications
- Reduce rate of myocardial infarction (MI)
- Stroke reduction in non-ST-segment elevation ACS
- Reduce rate of MI and stroke in acute ST-elevation myocardial infarction (STEMI) patients
- Reduce rate of MI and stroke in patients with established peripheral arterial disease
- Recent MI and recent stroke risk reduction
Pharmacology
- Prodrug
- Inhibition of platelet activation
- Irreversible binding to P2Y12 class of ADP receptors
- Dose-dependent inhibition of platelet aggregation
- Inhibition of platelet aggregation seen 2 hours after single oral doses
Dosage & Administration
- 300 mg oral loading dose for patients needing antiplatelet effect within hours
- 75 mg once daily orally for recent MI, recent stroke, or established peripheral arterial disease
- Oral administration with or without food
Interaction
- Increased risk of bleeding with NSAIDs, warfarin, SSRIs, SNRIs
- Avoid concomitant use of CYP2C19 inhibitors (omeprazole or esomeprazole)
- Increased plasma concentrations of Repaglinide with concomitant use
Contraindications
- Hypersensitivity to the drug substance or any component
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Common side effects: Bleeding, Diarrhoea, gastrointestinal discomfort, haemorrhage, Skin reactions
- Rare side effects: Acquired haemophilia, anaemia, angioedema, arthralgia, arthritis, bone marrow disorders
Pregnancy & Lactation
- Use during pregnancy if clearly needed and under medical supervision
- Unknown excretion of clopidogrel in human breast milk
Precautions & Warnings
- Impaired metabolism by genetic variations in CYP2C19
- Increased risk of bleeding due to lifetime platelet aggregation inhibition
- Increased risk of cardiovascular events on discontinuation
- Reported Thrombotic Thrombocytopenic Purpura (TTP)
- Reported hypersensitivity reactions
Use in Special Populations
- Safety and effectiveness not established in pediatric populations
- No dosage adjustment necessary in elderly patients

- May lead to bleeding complications
