Description

Indications
- Major Depressive Disorder (MDD)
- Generalized Anxiety Disorder (GAD)
- Diabetic Peripheral Neuropathic Pain (DPNP)
- Fibromyalgia
- Chronic Musculoskeletal Pain
Pharmacology
- Selective serotonin and norepinephrine reuptake inhibitor (SSNRI)
- No significant affinity for various receptors
- No inhibition of monoamine oxidase (MAO)
Dosage & Administration
- Starting Dose: 30-60 mg/day
- Target Dose: 30-60 mg/day for various conditions
- Maximum Dose: 120 mg/day
Interaction
- Metabolism via CYP1A2 and CYP2D6 isozymes
- Inhibition by fluvoxamine and similar drugs
- Higher concentrations with potent CYP2D6 inhibitors
Contraindications
- Known hypersensitivity to the drug or inactive ingredients
- Not approved for treating bipolar depression
- Not for patients with substantial alcohol use or chronic liver disease
- Cautious use in patients with controlled narrow-angle glaucoma

Precautions & Warnings
- Close monitoring for clinical worsening and suicidality
- Blood pressure measurement prior to treatment and periodically
- Caution with concomitant use of NSAIDs, aspirin, or other coagulation-affecting drugs
- Cautious use in patients with history of mania or seizure disorder
Use in Special Populations
- Safety and efficacy not established in pediatric patients
Overdose Effects
- Limited clinical experience with overdose

- Assurance of adequate airway, oxygenation, and ventilation
- Monitoring of cardiac rhythm and vital signs
