Indications

  • Treatment of type 2 diabetes mellitus in adults and children from 10 years of age
  • Reduction of diabetic complications in overweight type 2 diabetic adult patients

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers both basal and postprandial plasma glucose
  • Decreases hepatic glucose production
  • Improves insulin sensitivity

Dosage & Administration

  • Individualized dosage based on effectiveness and tolerance
  • Usual starting dose for adults: 500 mg twice a day or 850 mg once a day
  • Usual starting dose for children: 500 mg twice a day
  • Renal impairment precautions
  • Swallowing extended release tablet whole

Interaction

  • Potential interaction with certain drugs like Furosemide, Nifedipine, and others
  • Certain drugs may produce hyperglycemia and affect glycemic control

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure (GFR < 30 mL/min)
  • Conditions altering renal function, tissue hypoxia, and hepatic insufficiency

Pregnancy & Lactation

  • Increased risk of congenital abnormalities and perinatal mortality when uncontrolled diabetes
  • Not recommended during breastfeeding due to limited data

Precautions & Warnings

  • Excretion by the kidney, requiring regular monitoring
  • Lowering of vitamin B12 levels
  • Increased risk of hypoglycemia with insulin combination
  • Cautious use in elderly subjects and children aged between 10 and 12 years

Use in Special Populations

  • Dosage adjustment based on renal function in elderly subjects
  • Careful follow-up of metformin effect on growth and puberty in children
  • Regular assessment of renal function
  • Contradicted in patients with GFR < 30 ml/min

Overdose Effects

  • Lactic acidosis is a medical emergency
  • Hypoglycemia not seen with Xelmet doses up to 85 gm
  • Hemodialysis for effective removal of lactate and Xelmet
