Indications
- Treatment of major depressive disorder (MDD)
Pharmacology
- Enhances central noradrenergic and serotonergic activity
- Antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
- Antagonist of 5-HT2 and 5-HT3 receptors
- Antagonist of histamine (H1) receptors
- Moderate peripheral α1-adrenergic antagonist
- Moderate antagonist at muscarinic receptors
- Rapid and well-absorbed after oral administration
- Metabolized and eliminated via urine and faeces
- Displays linear pharmacokinetics within the recommended dose range
Dosage & Administration
- Recommended starting dose of 15 mg/day
- Administered in a single dose, preferably in the evening or prior to sleep
- Effective dose range of 15 to 45 mg/day
- Children are not recommended to Mirtazapine
- Instructions for missed dose
Interaction
- Clinically significant drug-drug interactions with different medications such as MAOI, tryptophan, triptans, linezolid, serotonin reuptake inhibitors, venlafaxine, lithium, tramadol, St. John's wort, Carbamazepine, Phenytoin, Cimetidine, Alcohol, and Diazepam
Contraindications
- Contraindicated in patients with hypersensitivity to Mirtazapine or any of the excipients
- Concomitant use of Mirtazapine and a monoamine oxidase (MAO) inhibitor is contraindicated
- Not to be used within 14 days of initiating or discontinuing therapy with MAOI

Pregnancy & Lactation
- Pregnancy Category-C
- Patients should notify their physician if they become pregnant or intend to become pregnant during Mirtazapine therapy
- Patients should notify their physician if they are breastfeeding an infant
Precautions & Warnings
- Patients, families, and caregivers should be alert to the emergence of certain behaviors and changes in behavior
- Risk of developing agranulocytosis
- May impair judgment, thinking, and motor skills
- Clinically significant ALT (SGPT) elevations may occur
