Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Enhances central noradrenergic and serotonergic activity
  • Antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
  • Antagonist of 5-HT2 and 5-HT3 receptors
  • Antagonist of histamine (H1) receptors
  • Moderate peripheral α1-adrenergic antagonist
  • Moderate antagonist at muscarinic receptors
  • Rapid and well-absorbed after oral administration
  • Metabolized and eliminated via urine and faeces
  • Displays linear pharmacokinetics within the recommended dose range

Dosage & Administration

  • Recommended starting dose of 15 mg/day
  • Administered in a single dose, preferably in the evening or prior to sleep
  • Effective dose range of 15 to 45 mg/day
  • Children are not recommended to Mirtazapine
  • Instructions for missed dose

Interaction

  • Clinically significant drug-drug interactions with different medications such as MAOI, tryptophan, triptans, linezolid, serotonin reuptake inhibitors, venlafaxine, lithium, tramadol, St. John's wort, Carbamazepine, Phenytoin, Cimetidine, Alcohol, and Diazepam

Contraindications

  • Contraindicated in patients with hypersensitivity to Mirtazapine or any of the excipients
  • Concomitant use of Mirtazapine and a monoamine oxidase (MAO) inhibitor is contraindicated
  • Not to be used within 14 days of initiating or discontinuing therapy with MAOI

Pregnancy & Lactation

  • Pregnancy Category-C
  • Patients should notify their physician if they become pregnant or intend to become pregnant during Mirtazapine therapy
  • Patients should notify their physician if they are breastfeeding an infant

Precautions & Warnings

  • Patients, families, and caregivers should be alert to the emergence of certain behaviors and changes in behavior
  • Risk of developing agranulocytosis
  • May impair judgment, thinking, and motor skills
  • Clinically significant ALT (SGPT) elevations may occur
