Indications
- Reduction of myocardial infarction (MI)
- Reduction of stroke in non-ST-elevation ACS
- Reduction of stroke in acute ST-elevation myocardial infarction (STEMI)
- Reduction of myocardial infarction and stroke in patients with peripheral arterial disease
- Reduction of MI and stroke in patients with recent MI or stroke
- Reduction of MI and stroke in patients with established peripheral arterial disease
Pharmacology
- Clopidogrel acts as a prodrug
- Inhibits platelet activation
- Irreversible binding to P2Y12 class of ADP receptors on platelets
- Dose-dependent inhibition of platelet aggregation
- Inhibition of platelet aggregation can be seen 2 hours after single oral doses
- Inhibition reaches steady state between Day 3 and Day 7
Dosage & Administration
- 300 mg oral loading dose for acute coronary syndrome
- 75 mg once daily orally for recent MI, stroke, or peripheral arterial disease
- Oral administration with or without food
Interaction
- Increased risk of bleeding with NSAIDs, warfarin, SSRIs, SNRIs
- Avoid concomitant use of omeprazole or esomeprazole
- Increased plasma concentrations of Repaglinide with concomitant use
Contraindications
- Hypersensitivity to the drug substance
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Common: Bleeding, Diarrhoea, gastrointestinal discomfort, haemorrhage, Skin reactions
- Rare: Acquired haemophilia, anaemia, angioedema, arthralgia, arthritis, bone marrow disorders
Pregnancy & Lactation
- Use during pregnancy only if clearly needed
- Unknown excretion in human breast milk
Precautions & Warnings
- Impaired metabolism in CYP2C19 poor metabolizer
- Increased risk of bleeding
- Increased risk of cardiovascular events upon discontinuation
- Reported cases of Thrombotic Thrombocytopenic Purpura (TTP)
- Reported cases of hypersensitivity including rash, angioedema or hematologic reaction
Use in Special Populations
- Safety and effectiveness not established in pediatric populations
- No dosage adjustment necessary in elderly patients

- May lead to bleeding complications
