Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Mechanism of action is unknown
  • Enhances central noradrenergic and serotonergic activity
  • Antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
  • Antagonist of 5- HT<sub>2</sub> and 5-HT<sub>3</sub> receptors
  • Potent antagonist of histamine (H<sub>1</sub>) receptors
  • Moderate peripheral &alpha;1-adrenergic antagonist
  • Moderate antagonist at muscarinic receptors
  • Rapid and well-absorbed after oral administration
  • Mean half-life of elimination is 20-40 hours
  • Extensively metabolized and eliminated via the urine and faeces

Dosage & Administration

  • Recommended starting dose of 15 mg/day
  • Single dose preferably in the evening or prior to sleep
  • Effective dose range of 15 to 45 mg/day
  • Dose changes should be made at intervals of less than 1 to 2 weeks
  • Use in children not recommended
  • Guidance on missed doses

Interaction

  • Clinically significant interactions with MAOIs and other serotonergic drugs
  • Interactions with Carbamazepine, Phenytoin, Cimetidine, Alcohol, and Diazepam

Contraindications

  • Contraindicated in patients with hypersensitivity to Mirtazapine or excipients
  • Concurrent use with MAO inhibitors

Side Effects

  • Common side effects include dizziness, drowsiness, dry mouth, increased appetite, weight gain
