Indications
- Management of hypertension for end-organ protection
- Useful in elderly patients and those with diabetes and albuminuria
- Use in patients with chronic kidney disease
Pharmacology
- Dihydropyridine calcium-channel blocker
- Binds to dihydropyridine binding sites of L-type voltage dependent calcium channel
- Inhibits Ca2+ influx across cell membranes of vascular smooth muscle cells
- Causes vasodilation and relaxes vascular smooth muscle
- Inhibits Ca2+ influx via N-type voltage dependent calcium channels in sympathetic nerve cell membrane
Dosage & Administration
- Adults: 5-10 mg once daily after breakfast
- Maximum dose: 20 mg once daily
- Pediatric use: Safety not established
- Elderly use: Initiate therapy with lowest possible dose (5 mg)

- Other anti-hypertensive drugs
- Antipsychotics causing hypotension
- Quinidine
- Carbamazepine
- Phenytoin
- Rifampicin
- Cimetidine
- Erythromycin
Contraindications
- Known sensitivity to Cilnidipine or excipients
- Cardiogenic shock
- Recent MI
- Acute unstable angina
- Severe aortic stenosis
Side Effects
- Dizziness
- Flushing
- Headache
- Hypotension
- Peripheral edema
- Palpitations
- GI disturbances
- Increased micturition frequency
- Lethargy
- Eye pain
- Depression
Pregnancy & Lactation
- Avoid administration in pregnant or possibly pregnant women
- Avoid administration to nursing mothers
- Discontinue lactation if administration is indispensable
Precautions & Warnings
- Administer with care in patients with serious hepatic dysfunction
- Administer with care in patients with history of serious adverse reactions to calcium antagonists
- Gradually decrease dosage during discontinuation under close observation
