Indications
- Reduction of myocardial infarction rate
- Reduction of stroke rate
- Treatment of acute coronary syndrome
- Treatment of recent MI
- Treatment of recent stroke
- Treatment of peripheral arterial disease
Pharmacology
- Prodrug
- Inhibition of platelet activation
- Irreversible binding to P2Y12 class of ADP receptors
- Dose-dependent inhibition of platelet aggregation
Dosage & Administration
- Initial loading dose of 300mg
- Maintenance dose of 75mg daily
- Oral administration
- With or without food
Interaction
- Increased bleeding risk with NSAIDs and warfarin
- Avoid concomitant use with CYP2C19 inhibitors like omeprazole
- Avoid concomitant use with repaglinide
- Increased risk of bleeding with selective serotonin and serotonin norepinephrine reuptake inhibitors (SSRIs, SNRIs)
Contraindications
- Hypersensitivity to the drug substance
- Active pathological bleeding
Side Effects
- Common side effects: bleeding, diarrhea, gastrointestinal discomfort, skin reactions
- Rare side effects: acquired haemophilia, anaemia, angioedema, bone marrow disorders
Pregnancy & Lactation
- Use during pregnancy only if clearly needed
- Unknown excretion in human breast milk
Precautions & Warnings
- Metabolism impairment by genetic variations in CYP2C19
- Increased risk of bleeding
- Discontinuation increases risk of cardiovascular events
- Thrombotic Thrombocytopenic Purpura (TTP) reported
- Possible hypersensitivity reactions
Use in Special Populations
- Safety and effectiveness not established in pediatric populations
- No dosage adjustment necessary in elderly patients
Overdose Effects
- May lead to bleeding complications
- Platelet transfusion may restore clotting ability
