Indications
- Monotherapy in the treatment of partial-onset seizures
- Adjunctive therapy in the treatment of myoclonic seizures
- Adjunctive therapy in the treatment of primary generalized tonic-clonic seizures
- Treatment of epilepsy in adults and adolescents
- Treatment of epilepsy in children and infants from 1 month of age
Mechanism of Action
- Precise mechanism(s) unknown
- Minimal activity in submaximal stimulation
- Protection against secondarily generalized activity
- Inhibition of burst firing
- Selective prevention of hyper synchronization
Dosage
- Recommended starting dose of 250 mg twice daily for adults and adolescents from 16 years of age
- Maximum dose of 1500 mg twice daily for adults and adolescents
- Dose adjustments for pediatric population based on weight and age
- Use of oral solution for infants and children under 6 years
Administration
- Oral administration
- Swallowed with a sufficient quantity of liquid
- Administered in two equally divided doses
- Can be taken with or without food
Interaction
- No influence on serum concentrations of existing antiepileptic medicinal products
- Inhibition of renal clearance by probenecid
- Decrease in methotrexate clearance
- Decreased efficacy with concomitant use of osmotic laxative macrogol
- No alteration of absorption by food, but reduced rate of absorption
Contraindications
- Hypersensitivity to the active substance or other pyrrolidone derivatives
- Hypersensitivity to any of the excipients
Side Effects
- Most common adverse reactions include somnolence, asthenia, infection, dizziness in adults
- Most common adverse reactions include fatigue, aggression, nasal congestion, decreased appetite, irritability in pediatric patients
Precautions
- Dose adjustment may be required for patients with renal impairment
- Association with acute kidney injury
- Advice for blood cell counts
- Monitoring for signs of depression and suicidal ideation and behaviors
- Caution for use in infants and children below 6 years
Pregnancy & Lactation
- Not recommended during pregnancy and in women of childbearing potential not using contraception
- Excreted in human breast milk, breastfeeding not recommended
Use in Special Populations
- Dose adjustment recommended in elderly patients with compromised renal function
Overdose Effects
- Observation of somnolence, agitation, aggression, depressed level of consciousness, respiratory depression and coma
- Symptomatic treatment for overdose, including haemodialysis
