Description
- Indicated for the acute and maintenance treatment of schizophrenia and related psychoses
- Treatment of acute manic or mixed episodes in bipolar disorder
- Has affinities for serotonin, dopamine, cholinergic muscarinic receptors, adrenergic, and histamine receptors
- Well absorbed after oral administration
- Not mutagenic or clastogenic
- Contraindicated in patients with known risk for narrow-angle glaucoma
- Should be used cautiously in patients with low leukocyte and/or neutrophil counts
- Keep below 30°C temperature, away from light & moisture
Indications
- Treatment of acute and maintenance of schizophrenia and related psychoses
- Treatment of acute manic or mixed episodes in bipolar disorder
Pharmacology
- Affinities for serotonin 5HT2A/2C, 5HT3, 5HT6; dopamine D1, D2, D3, D4, D5; cholinergic muscarinic receptors M1-M5; α1 adrenergic; and histamine H1 receptors
- Unknown mechanism of action
- Well absorbed after oral administration
Dosage
- Initial dose is 5-10 mg once daily for schizophrenia and bipolar disorder combination therapy
- Initial dose is 15 mg once daily for bipolar disorder monotherapy
- Maximum dose is 20 mg daily
Administration
- Can be given without regard to meals
- Gradual tapering of the dose should be considered while discontinuing
Interaction
- May antagonize the effects of levodopa and dopamine agonists
- May be affected by drugs that induce or inhibit CYP1A2 or glucoronyl transferase enzymes
- Carbamazepine may increase the clearance of Olanap ODT
- Activated charcoal reduces the oral bioavailability of Olanap ODT by 50-60%
- Caution should be taken when administered with centrally acting drugs and alcohol
Contraindications
- Known hypersensitivity to any ingredient of the product
- Known risk for narrow-angle glaucoma
Side Effects
- Very common undesirable effects include somnolence and weight gain
- Other effects include increased appetite, elevated glucose levels, elevated triglyceride levels, dizziness, akathisia, Parkinson's disease, dyskinesia, orthostatic hypotension, mild and transient anticholinergic effects, asthenia, edema, and photosensitivity reaction
Pregnancy & Lactation
- Should be used in pregnancy only if the potential benefits justify the potential risk to the fetus
- No reports of teratogenicity
- Patients should not breastfeed while taking Olanzapine
Precautions & Warnings
- Use cautiously in patients with history of seizures, low leukocyte and/or neutrophil counts, drug-induced bone marrow depression/toxicity, hypereosinophilic conditions, impaired hepatic function, and those using hepatotoxic medicines or centrally acting drugs
- Patients should be cautioned about operating hazardous machinery, including motor vehicles
Use in Special Populations
- Not studied in subjects under 18 years of age
- Starting dose for elderly patients (age 65 and over) is 5 mg/day
- Starting dose for patients with hepatic or renal impairment is 5 mg/day
