Indications
- Acute and chronic psychoses
- Mania
- Schizophrenia
Pharmacology
- Selective monoaminergic antagonist
- High affinity for serotoninergic 5-HT2 and dopaminergic D2 receptors
- Binds to alpha1 adrenergic receptors
- Binds with lower affinity to H1 histamine and alpha2 adrenergic receptors
- No affinity for cholinergic receptors
- Balanced central serotonin and dopamine antagonism
Dosage & Administration
- Initial dose of 2 mg on 1st day, then 4 mg on 2nd day
- Usual daily dose range 4-6 mg
- Maximum dose of 16 mg daily
- Special dosage considerations for elderly or patients with hepatic or renal impairment
- Child under 15 years not recommended
- Initial dose of 2 mg once daily for mania
- Usual daily dose range for mania 1-6 mg
- Initial dose of 1 mg BID for schizophrenia
- Target dose of 3 mg BID for schizophrenia
- Dosage adjustments at intervals of not less than 1 week
Interaction
- Antagonizes the effects of levodopa and dopamine agonists
- Reduces plasma clearance with chronic administration of Carbamazepine
- Decreases the clearance with chronic administration of Clozapine
- Enhances the effects of certain antihypertensives
