Description

  • Clocard-A is indicated for reducing the rate of Myocardial Infarction (MI) and Stroke in patients with non-ST-segment elevation ACS and acute ST-segment elevation ACS. It is also used to reduce the rate of MI and Stroke in patients with recent MI, recent Stroke, or established Peripheral Arterial Disease. Clocard-A inhibits platelet activation and aggregation through irreversible binding of its active metabolite to the P2Y12 class of ADP receptors on platelets, and also inhibits platelet aggregation by irreversible inhibition of platelet cyclooxygenase and thereby inhibiting the generation of thromboxane A2.

Dosage and Administration

  • Recommended dose of 75 mg once daily with or without food. For patients with acute coronary syndrome (unstable angina/non-Q-wave MI), it should be initiated with a single 300 mg loading dose and then continued at 75 mg once daily. Aspirin should be initiated and continued in combination with Clocard-A.

Interaction

  • Clocard-A does not modify the Clopidogrel-mediated inhibition of ADP-induced platelet aggregation. It potentiates the effect of aspirin on collagen-induced platelet aggregation. Co-administration with heparin has no effect on inhibition of platelet aggregation induced by Clocard-A. Concomitant administration with NSAIDs is associated with increased occult gastrointestinal blood loss. The safety of the co-administration of Clocard-A with warfarin has not been established.

Contraindications

  • Hypersensitivity to the drug substance or any component of the product. Active pathological bleeding such as peptic ulcer or intracranial hemorrhage.

Side Effects

  • Clocard-A is generally well tolerated.

Pregnancy and Lactation

  • Should be used during first and second trimesters of pregnancy only if clearly needed. Contraindicated during the third trimester of pregnancy. It is unknown whether Clopidogrel is excreted in human breast milk but Aspirin is known to be excreted in human milk. This drug should be discontinued during breastfeeding.

Precautions and Warnings

  • May prolong bleeding time. TTP and Reye's syndrome have been reported rarely following use of Clocard-A. Hypersensitivity including rash, angioedema or hematologic reaction has been reported in patients receiving Clocard-A or history of hypersensitivity to other thienopyridines.

Use in Special Populations

  • Should not be given to children, particularly those under 12 years, unless the expected benefits outweigh the possible risks. Aspirin may be a contributory factor in the causation of Reye’s syndrome in some children.

Overdose Effects

  • Clopidogrel overdose may lead to bleeding complications. Platelet transfusion may restore clotting ability. Aspirin overdose can lead to symptoms such as dizziness, headache, tinnitus, confusion, and gastrointestinal symptoms. Treatment may include inducing vomiting followed by gastric lavage if needed. Severe Aspirin intoxication may lead to respiratory alkalosis, respiratory acidosis, metabolic acidosis, hyperthermia, perspiration, and dehydration. It can be treated with haemodialysis and other symptomatic treatment.
