Indications

  • Acute bacterial exacerbation of chronic bronchitis in adults

Pharmacology

  • Inhibits microsomal protein synthesis
  • Binds to 50S subunit of bacterial ribosome
  • Active against most Gram-positive bacteria
  • More acid-stable than Erythromycin
  • Twice the activity of Erythromycin against H. influenzae
  • Inactivity against most species of Gram-negative bacteria

Interaction

  • Contraindicated with specific medications

Contraindications

  • Known hypersensitivity to clarithromycin or macrolide antibacterial drugs
  • History of cholestatic jaundice or hepatic dysfunction associated with prior use of clarithromycin

Precautions & Warnings

  • Avoid in patients with known QT prolongation or receiving drugs known to prolong the QT interval
  • Increased risk of all-cause mortality in patients with coronary artery disease
  • Evaluate for Clostridium difficile associated diarrhea if diarrhea occurs

  • Safety and effectiveness in pediatric patients
  • Safety and effectiveness for prevention of disseminated Mycobacterium avium complex (MAC) disease in pediatric patients with advanced HIV infection
