Indications
- Reduction of myocardial infarction rate
- Reduction of stroke rate
- Treatment for non-ST-segment elevation ACS
- Treatment for ST-elevation myocardial infarction (STEMI)
- Reduction of MI and stroke rate in patients with recent MI or recent Stroke
- Reduction of MI and stroke rate in patients with established Peripheral Arterial Disease
Pharmacology
- Prodrug
- Inhibition of platelet activation
- Irreversible binding of active metabolite to P2Y12 class of ADP receptors
- Dose-dependent inhibition of platelet aggregation
- Onset of action within 2 hours after single oral doses
Dosage & Administration
- Single 300 mg oral loading dose for patients who need antiplatelet effect within hours
- Maintenance dose of 75 mg orally once daily
- Oral administration with or without food
Interaction
- Increased risk of bleeding with NSAIDs, warfarin, SSRIs, SNRIs
- Avoid concomitant use of CYP2C19 inhibitors (omeprazole or esomeprazole)
- Increased plasma concentrations of Repaglinide when used with Preclot
Contraindications
- Hypersensitivity to the drug substance or any component
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Common: Bleeding, Diarrhoea, gastrointestinal discomfort, haemorrhage, Skin reactions
- Rare: Acquired haemophilia, anaemia, angioedema, arthralgia, arthritis, bone marrow disorders
Pregnancy & Lactation
- Use during pregnancy only if clearly needed
- Unknown excretion of clopidogrel in human breast milk
- Decision making for nursing mothers

- Metabolism impairment by genetic variations in CYP2C19
- Inhibition of platelet aggregation for the lifetime of the platelet
- Increased risk of bleeding and restoration of hemostasis
- Discontinuation risk and timing for elective surgery
- Thrombotic Thrombocytopenic Purpura (TTP) risks
- Hypersensitivity reactions
