Medicine Details

Indications

  • Prophylaxis and chronic treatment of asthma
  • Acute prevention of Exercise-Induced Bronchoconstriction (EIB)
  • Relief of symptoms of Allergic Rhinitis (AR): Seasonal & Perennial Allergic Rhinitis

Pharmacology

  • Selective and orally active leukotriene receptor antagonist
  • Inhibits the cysteinyl leukotriene receptor (CysLT1)
  • Correlated with the pathophysiology of asthma & allergic rhinitis

Dosage & Administration

  • Adults and adolescents dosage: Montelukast 10 mg tablet once daily
  • Pediatric patients dosage: Montelukast 5 mg to 4 mg tablet once daily
  • Safety and efficacy in pediatric patients established
  • No dosage adjustment in patients with hepatic or renal insufficiency
  • Similar pharmacokinetic profile and oral bioavailability in elderly and younger adults

Interaction

  • Administered with other asthma therapies with no increase in adverse reactions
  • No clinically important effects on the pharmacokinetics of certain drugs
  • Used concomitantly with a range of commonly prescribed drugs without evidence of clinical adverse interactions
  • Phenobarbital decreased the AUC of Arovent approximately 40%
  • No dosage adjustment recommended for Arovent
  • Appropriate clinical monitoring recommended when co-administered with potent cytochrome P450 enzyme inducers

Contraindications

  • Contraindicated in patients hypersensitive to any component

Side Effects

  • Common side effects: Diarrhea, fever, headache, nausea, vomiting, upper respiratory tract infection
  • Uncommon side effects: Anxiety, depression, dizziness, dry mouth, muscle complaints, sleep disorders
  • Rare side effects: Angioedema, hallucination, memory loss, palpitations

Pregnancy & Lactation

  • Crosses the placenta in rats and rabbits
  • Use during pregnancy only if clearly needed
  • Caution should be exercised when given to a nursing mother

Precautions & Warnings

  • Not indicated for use in the reversal of bronchospasm in acute asthma attacks
  • Appropriate rescue medication should be available for acute exacerbations of asthma
  • Not to be abruptly substituted for inhaled or oral corticosteroids
  • Not to be used as monotherapy for the treatment of exercise-induced bronchospasm
  • Caution for patients with known aspirin sensitivity
  • Not shown to truncate bronchoconstrictor response to aspirin in aspirin-sensitive asthmatic patients

Overdose Effects

  • No adverse experiences in the majority of overdosage reports
  • Usual supportive measures should be employed in the event of overdose
